目的:探讨和评价螺旋CT三维成像技术在眼眶爆裂性骨折修复术中的必要性、有效性以及对手术的临床指导作用。方法:以2D-CT、多平面重建(MPR)及3D-CT成像技术诊断眶壁骨折,以MEDPOR人工骨片及羟基磷灰石(HA)人工骨片为填充材料,对86例眼眶爆裂性骨折患者施行眶壁骨折整复术。结果:2D-CT良好的显示出眶内软组织嵌顿及疝出的状态,多平面重建(MPR)及3D-CT清晰的显示出眶壁骨折空间立体结构,窦腔立体重建图像具有帮助术前诊断和了解副鼻窦损伤情况的作用。结论:螺旋CT三维成像有助于医生在术前全面整体基本量化了解眶壁骨折的位置和范围,构成手术中的立体视觉参考模型,以便精确的进行手术设计和指导手术过程。

Objective:To discuss and evaluate necessity, effectiveness and clinical directive function of spiral computed tomograph three-dimensional imaging in the surgery procedures of orbital fracture repair. Methods Obtital fractures were diagnosed by the technique of two-dimensional computed tomography, multiplanar reconstruction and three-dimensional computed tomography. Medpor or HA artificial bone were chosen as the filling material. Orbital fracture repair were executed in the 86 patients. Results The incarcerated and herniated orbital soft tissues were displayed clearly in the two-dimensional computed tomography image, multiplanar reconstruction and three-dimensional computed tomography images could show accurately the three-dimensional space structure of orbital fractures. The stereoscopic reconstructed images of sinus cavity could help preoperative diagnosis and acquaintance of injury of paranasal sinuses. slightly in twenty-four patients and aggravated in two patients after operations. Conclusions Spiral computed tomograph three-dimensional imaging could help doctors knowledge position and scope of orbital wholely and quantitively before operation, form stereo visual reference model in the surgery procedures of orbital fracture repair. Therefore, operations could be devised and directed accurately.
