目的:探讨不同方法建立兔室壁瘤模型特点及其应用价值。方法新西兰白兔80只随机法分为4组,每组20只。根据不同开胸方式和结扎不同冠脉建立兔室壁瘤动物模型。胸骨开胸左前降支组采用剪开正中胸骨开胸,结扎兔左前降支动脉中段;胸骨开胸左前降支及左旋支组采用剪开正中胸骨开胸,结扎兔左前降支和左旋支动脉中段;肋间隙开胸左前降支组采用左侧胸腔第3~4肋间隙开胸,结扎兔左前降支动脉中段;肋间隙开胸左前降支及左旋支组采用左侧胸腔第3~4肋间隙开胸,结扎兔左前降支及左旋支动脉中段。饲养4w 后,应用实时三维超声心动图进行模型评价,比较不同开胸方式和结扎不同冠脉建立兔室壁瘤模型的成功率、存活率及室壁瘤形成特点。结果(1)胸骨开胸左前降支组、肋间隙开胸左前降支组动物存活率(90%、95%)明显高于胸骨开胸左前降支及左旋支组与肋间隙开胸左前降支及左旋支组(65%、70%)( P <0.05)。(2)胸骨开胸左前降支及左旋支组、肋间隙开胸左前降支及左旋支组室壁瘤形成率(84%、86%)明显高于胸骨开胸左前降支组与肋间隙开胸左前降支组(50%、53%)( P<0.05)。(3)胸骨开胸左前降支及左旋支组、肋间隙开胸左前降支及左旋支组左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、室壁瘤容积(LVAV)较胸骨开胸左前降支组与肋间隙开胸左前降支组明显增加(P <0.05),LVEF明显下降(P <0.05)。结论结扎不同冠状动脉对建立室壁瘤模型动物存活率和造模成功率有直接的影响,结扎左前降支和左旋支建立室壁瘤模型的模型成功率较高,可形成容积较为固定的室壁瘤。

목적:탐토불동방법건립토실벽류모형특점급기응용개치。방법신서란백토80지수궤법분위4조,매조20지。근거불동개흉방식화결찰불동관맥건립토실벽류동물모형。흉골개흉좌전강지조채용전개정중흉골개흉,결찰토좌전강지동맥중단;흉골개흉좌전강지급좌선지조채용전개정중흉골개흉,결찰토좌전강지화좌선지동맥중단;륵간극개흉좌전강지조채용좌측흉강제3~4륵간극개흉,결찰토좌전강지동맥중단;륵간극개흉좌전강지급좌선지조채용좌측흉강제3~4륵간극개흉,결찰토좌전강지급좌선지동맥중단。사양4w 후,응용실시삼유초성심동도진행모형평개,비교불동개흉방식화결찰불동관맥건립토실벽류모형적성공솔、존활솔급실벽류형성특점。결과(1)흉골개흉좌전강지조、륵간극개흉좌전강지조동물존활솔(90%、95%)명현고우흉골개흉좌전강지급좌선지조여륵간극개흉좌전강지급좌선지조(65%、70%)( P <0.05)。(2)흉골개흉좌전강지급좌선지조、륵간극개흉좌전강지급좌선지조실벽류형성솔(84%、86%)명현고우흉골개흉좌전강지조여륵간극개흉좌전강지조(50%、53%)( P<0.05)。(3)흉골개흉좌전강지급좌선지조、륵간극개흉좌전강지급좌선지조좌실서장말기용적(LVEDV)、좌실수축말기용적(LVESV)、실벽류용적(LVAV)교흉골개흉좌전강지조여륵간극개흉좌전강지조명현증가(P <0.05),LVEF명현하강(P <0.05)。결론결찰불동관상동맥대건립실벽류모형동물존활솔화조모성공솔유직접적영향,결찰좌전강지화좌선지건립실벽류모형적모형성공솔교고,가형성용적교위고정적실벽류。
Objective To assess the different method for establishment of rabbit model with left ventricular aneurysm (LVA) by ligating different coronary artery and different thoracotomy .Methods Eighty New Zealand rabbits were divided into four groups .Sternum thoracotomy and left anterior descending artery (STLAD) group :Rabbit model of LVA was prepared by ligating the middle segment of the left anterior descending artery and thoracotomy by cutting the middle sternum .ternum thoracotomy and left anterior descending and left circumflex artery (STLDCA ) group:Rabbit model of LVA was prepared by ligating the middle segment of the left anterior descending artery and the left circumflex artery and thoracotomy by cutting the middle sternum .Intercostal space thoracotomy and left anterior descending artery (ISLAD ) group :Rabbit model of LVA was prepared by ligating the middle segment of the left anterior descending artery and thoracotomy was performed through the left fourth intercostal space .Iintercostal space thora-cotomy and left anterior descending and left circumflex artery (ISLADCA ) Group:Rabbit model of LVA was prepared by ligating the middle segment of the left anterior descending artery and the left circumflex artery and thoracotomy was performed through the left fourth intercostal space .At 4 weeks after the pro-cedure ,real-time three-dimensional echocardiography (RT-3DE) was conducted to obtain data on LVEF , left ventricular end-diastolic volume (LVEDV) ,left ventricular end-systolic volume (LVESV) ,and LVA volume ,animal livability and the formation rate of LVA were compared with four group .Results (1) The animal livability in group STLAD ,ISLAD were significantly higher than that in group STLDCA and IS-LADCA (P <0 .05) .(2) The formation rate of LVA in group STLDCA and ISLADCA were significantly higher than that in group STLAD ,ISLAD (P <0 .05) .(3) Compared with group STLAD ,ISLAD ,LV-EDV ,LVESV ,and LVA volume were significantly enlarged in group STLDCA and ISLADCA ,while LVEF was significantly decreased (P<0 .05) .Conclusion Ligating the left anterior descending artery and the left circumflex artery plays the important role in the livability of animals and formation rate of LVA af-ter AMI .RT-3DE can be as a new method to accurately measure the LVA volume and evaluate the LVA morphology .
