目的：通过检测分析血清及脑脊液中肿瘤坏死因子-ɑ（ TNF-ɑ）及其受体在病毒性脑炎患儿中治疗前后的浓度变化，探寻TNF-ɑ在病毒性脑炎发病中的作用机制。方法：对象为本院患者中随机选取的15例重症与10例轻症患病毒性脑炎患儿和10例股疝患儿。用ELISA方法在治疗前测定血清和脑脊液以及在治疗后血清当中TNF-ɑ的浓度并进行比较分析。结果：患儿血清和脑脊液中TNF-ɑ浓度在轻症组和重症组中和治疗前相比明显升高，升高程度重症组大于轻症组，轻症组大于对照组（p＜0．01），差异有统计学意义。治疗后，重症组和轻症组患儿中血清TNF-ɑ浓度与治疗前相比明显下降（p＜0．01），统计学有显著差异。结论：病毒性脑炎患儿血清和脑脊液中TNF-ɑ水平与疾病严重程度呈正相关，治疗前浓度越高病情越严重。
목적：통과검측분석혈청급뇌척액중종류배사인자-ɑ（ TNF-ɑ）급기수체재병독성뇌염환인중치료전후적농도변화，탐심TNF-ɑ재병독성뇌염발병중적작용궤제。방법：대상위본원환자중수궤선취적15례중증여10례경증환병독성뇌염환인화10례고산환인。용ELISA방법재치료전측정혈청화뇌척액이급재치료후혈청당중TNF-ɑ적농도병진행비교분석。결과：환인혈청화뇌척액중TNF-ɑ농도재경증조화중증조중화치료전상비명현승고，승고정도중증조대우경증조，경증조대우대조조（p＜0．01），차이유통계학의의。치료후，중증조화경증조환인중혈청TNF-ɑ농도여치료전상비명현하강（p＜0．01），통계학유현저차이。결론：병독성뇌염환인혈청화뇌척액중TNF-ɑ수평여질병엄중정도정정상관，치료전농도월고병정월엄중。
Objective:Via detecting level changes of tumor necrosis factor -ɑ( TNF-ɑ) and its receptor in serum and cerebrospinal fluid, validating the mechanism of TNF-ɑ in encephalitis .Methods:Encephalitis children involved 15 with severe ( SVE) and 10 with mild( MVE) , 10 children with femoral hernia were included .The levels of TNF-ɑwas detected before treatment using ELISA in serum and CSF of all children .Results:Before treatment , the TNF-ɑlevel in serum and CSF elevated significantly while compared with control group;The serum and CSF TNF-ɑlevel in SVE group was higher than MVE group was , the difference was significant(p<0.01).After treatment, TNF-ɑdecreased in both group of SVE and MVE compare with levels before treatment (<0.01).Conclusion:The TNF-ɑlevel in serum increased in children with VE , the results indicate that the serum and CSF TNF -ɑlevel has an positive correlation with the severity of the disease .
