目的：通过对87例大面积脑梗死患者的临床分析，总结大面积脑梗死的临床高危因素与诊疗经验。方法对87例大面积脑梗死患者的临床资料进行回顾性分析。结果高血压、糖尿病、心脏病伴发心律失常、高脂血症、吸烟等是大面积脑梗死致病的高危因素。结论对发病初期病人进行颅脑CT；控制同型半胱氨酸(Hcy)在血液中水平；预防肺部感染、降低颅内压、及早防止脑疝发生；畅通呼吸道，必要时使用呼吸机辅助呼吸；采用去骨瓣减压术等都是降低大面积脑梗死死亡率，提高治疗有效率的有效方法。
목적：통과대87례대면적뇌경사환자적림상분석，총결대면적뇌경사적림상고위인소여진료경험。방법대87례대면적뇌경사환자적림상자료진행회고성분석。결과고혈압、당뇨병、심장병반발심률실상、고지혈증、흡연등시대면적뇌경사치병적고위인소。결론대발병초기병인진행로뇌CT；공제동형반광안산(Hcy)재혈액중수평；예방폐부감염、강저로내압、급조방지뇌산발생；창통호흡도，필요시사용호흡궤보조호흡；채용거골판감압술등도시강저대면적뇌경사사망솔，제고치료유효솔적유효방법。
Objective To make an analysis on massive cerebral infarction in 87 cases to explore their clinical risk factors and efficient therapy methods.Methods We reviewed and analyzed 87 cases of massive cerebral infarction in cause and clinical diagnosis and treatment.Results We summed up the risk factors of massive cerebral infarction such as hypertension, diabetes mel itus, heart disease with arrhythmias, hyperlipidemia and smoking.Conclusion In therapy, there are key links to reduce the mortality and improve efficiency. For instance,onsetting brain CT to patients with early disease;control ing homocysteine (Hcy) levels in the blood;preventing lung infection;reducing intracranial pressure;preventing the occurrence of hernia as soon as possible;unimpeded respiratory tract,ventilator-assisted breathing when it's necessary;using decompressive craniectomy and so on.
