目的：探讨外阴恶性肿瘤传统手术方式与保留大隐静脉主干的改良手术方式对术后并发症的影响。方法：将2009年1月至2010年12月住院手术的外阴癌患者随机分为两组：10例行腹股沟淋巴结清扫术时保留双侧大隐静脉主干，10例结扎、切除一段大隐静脉，对两组患者的手术时间、术中出血量、术后急慢性并发症及复发率进行比较。结果：两组患者的手术时间、术中出血量、术后复发率差异均无显著性意义（P>0.05），切口愈合时间、下肢水肿、急性静脉炎等并发症的发生率在保留大隐静脉组明显的低于切除大隐静脉组（P<0.05）。结论：外阴恶性肿瘤行腹股沟淋巴结清扫术时保留双侧大隐静脉主干，不增加手术难度，不延长手术时间，不增加复发率，但是能明显的降低术后多种并发症的发生，能明显改善患者的生活质量。
목적：탐토외음악성종류전통수술방식여보류대은정맥주간적개량수술방식대술후병발증적영향。방법：장2009년1월지2010년12월주원수술적외음암환자수궤분위량조：10례행복고구림파결청소술시보류쌍측대은정맥주간，10례결찰、절제일단대은정맥，대량조환자적수술시간、술중출혈량、술후급만성병발증급복발솔진행비교。결과：량조환자적수술시간、술중출혈량、술후복발솔차이균무현저성의의（P>0.05），절구유합시간、하지수종、급성정맥염등병발증적발생솔재보류대은정맥조명현적저우절제대은정맥조（P<0.05）。결론：외음악성종류행복고구림파결청소술시보류쌍측대은정맥주간，불증가수술난도，불연장수술시간，불증가복발솔，단시능명현적강저술후다충병발증적발생，능명현개선환자적생활질량。
Objective To investigate the conventional operation and preserving the saphenous vein during inguinal lymphadenectomy for Vulvar carcinoma on operation and complications. Methods 20 patients with vulvar carcinoma,treated in xinxiang central hospital from Jan.2009 to Dec.2010,were divided into 2groups:the saphenous vein was preserved in 10 patients and ligated in 10 patients during inguinal lymphadenectomy,the operating time, intraoperative blood loos,recurrence rate and complications were compared between the groups.Results the operating time,intraoperative blood loos and recurrence rate were no statistically significant between two groups(P>0.05),the occurrence rates of acute low extremity lymph edema,low extremity phlebitis were lower in preservation group.Conclusion Preserving the saphenous vein during inguinal lymphadenectomy for vulvar carcinoma reduces multiple postoperative complications without increasing the operation difficulty,this modified operative modality is valuable to improve the quality of life of the vulvar carcinoma patients.
