目的：比较床边超声引导下应用改良赛丁格技术行PICC置管与传统PICC置管在困难置管中的效果差异。方法：80例拟实施经浅静脉PICC置管的患者，随机分为对照组(常规盲穿)和观察组(超声定位引导穿刺)，比较两组穿刺置管成功率、疼痛感及并发症发生率。结果：超声引导下较传统PICC置管对于困难血管可获得较高的一次性成功率，减少穿刺的痛苦，明显降低并发症的发生。结论：超声引导下改良赛丁格技术PICC置管，适合于外周静脉状况差，穿刺困难的患者，值得临床推广。
목적：비교상변초성인도하응용개량새정격기술행PICC치관여전통PICC치관재곤난치관중적효과차이。방법：80례의실시경천정맥PICC치관적환자，수궤분위대조조(상규맹천)화관찰조(초성정위인도천자)，비교량조천자치관성공솔、동통감급병발증발생솔。결과：초성인도하교전통PICC치관대우곤난혈관가획득교고적일차성성공솔，감소천자적통고，명현강저병발증적발생。결론：초성인도하개량새정격기술PICC치관，괄합우외주정맥상황차，천자곤난적환자，치득림상추엄。
Objective:Comparison of bedside ultrasound guided by modified Seldinger technique in PICC and traditional PICC catheterization in difficult catheter in the effect of differences. Methods:In 80 cases of the implementation of the superficial vein catheter in patients with PICC, Were randomly divided into control group (conventional blind) and observation group (ultrasound guided puncture), Comparison of two groups of puncture success rate, pain and the incidence of complications. Results:Under the guidance of ultrasound is better than traditional PICC catheter successfully obtains the higher for difficult vascular rate, reduce the pain, reduce complications. Conclusion:Ultrasound-guided modified Seldinger technique in patients with PICC catheter, especially suitable for peripheral venous conditions, its superiority is obvious, it is worthy of clinical application.
