目的:探讨BiPAP面罩式双水平正压通气治疗重症支气管哮喘的疗效。方法选择重症支气管哮喘患者68例,随机抽样分为治疗组(33例)和对照组(35例)。治疗组在常规药物治疗基础上行BiPAP面罩式双水平正压通气治疗;对照组予单纯常规药物治疗。观察两组治疗前后的临床症状和血气分析的变化情况。结果治疗组临床症状明显改善,血氧分压( PaO2)、血氧饱和度( SaO2)显著升高,二氧化碳分压( PaCO2)显著降低,与对照组比较差异有统计学意义( P<0.05)。结论重症支气管哮喘在常规治疗的基础上加用BiPAP面罩式双水平正压通气治疗,可有效改善患者的临床症状、血气分析指标,疗效满意,安全,值得临床推广应用。

목적:탐토BiPAP면조식쌍수평정압통기치료중증지기관효천적료효。방법선택중증지기관효천환자68례,수궤추양분위치료조(33례)화대조조(35례)。치료조재상규약물치료기출상행BiPAP면조식쌍수평정압통기치료;대조조여단순상규약물치료。관찰량조치료전후적림상증상화혈기분석적변화정황。결과치료조림상증상명현개선,혈양분압( PaO2)、혈양포화도( SaO2)현저승고,이양화탄분압( PaCO2)현저강저,여대조조비교차이유통계학의의( P<0.05)。결론중증지기관효천재상규치료적기출상가용BiPAP면조식쌍수평정압통기치료,가유효개선환자적림상증상、혈기분석지표,료효만의,안전,치득림상추엄응용。
Objective To investigate curative effect of noninvasive bi-level positive airway pressure ventila-tion on severe bronchial asthma .Methods Six-eight patients with severe bronchial asthma were randomly divided in-to 2 groups .The patients in 2 groups were given routine medication treatment after admission .While on the above ba-sis,the treatment group(n=33) received bi-level positive airway pressure ventilation .The clinical symptoms and the indexes of arterial blood gas were observed before and after treatment .Results After treatment , the clinical symp-toms,and the indexes of blood gas had significant improvement in the treatment group , PaCO2 decreased and PaO 2 and SaO2 increased in the treatment group .The improvement in PaCO 2 , PaO2 ,and SaO2 in the treatment group were better than that in the control group ( P<0.05 ) .Conclusion Treatment of noninvasive bi-level positive airway pres-sure ventilation is effective for treating servere bronchial asthma .
