目的:研究并分析吸烟对终末期肾病患者全段甲状旁腺激素(iPTH)水平的影响以及与左心室重塑的关系。方法根据吸烟情况将139例慢性肾小球肾炎终末期肾病非透析患者分组,测定各组的iPTH及其他临床化验指标,并计算各组左心室质量指数(LVMI)。分析吸烟对iPTH的影响以及与LVMI的相关性。结果吸烟组iPTH高于不吸烟组,重度吸烟组iPTH高于轻度吸烟组,吸烟与iPTH独立正相关,且iPTH随着吸烟量增加而升高,呈剂量依赖关系;有左心室肥厚组iPTH高于无左心室肥厚组,iPTH与LVMI独立正相关。结论血iPTH指标的提升是终末期肾病吸烟患者高发左心室肥厚症状的关键因素,值得临床重视。

목적:연구병분석흡연대종말기신병환자전단갑상방선격소(iPTH)수평적영향이급여좌심실중소적관계。방법근거흡연정황장139례만성신소구신염종말기신병비투석환자분조,측정각조적iPTH급기타림상화험지표,병계산각조좌심실질량지수(LVMI)。분석흡연대iPTH적영향이급여LVMI적상관성。결과흡연조iPTH고우불흡연조,중도흡연조iPTH고우경도흡연조,흡연여iPTH독립정상관,차iPTH수착흡연량증가이승고,정제량의뢰관계;유좌심실비후조iPTH고우무좌심실비후조,iPTH여LVMI독립정상관。결론혈iPTH지표적제승시종말기신병흡연환자고발좌심실비후증상적관건인소,치득림상중시。
Objective To study and analyse the ef ects of smoking on intact parathyroid hormone and the relationship between smoking and left ventricular remodeling in the end stage renal disease patients. Methods 139 end-stage renal disease of chronic glomerulonephritis non-dialysis patients were divided into groups according to the situation of Smoking. To detecte the patients’s iPTH and other clinical laboratory indicators in each group, and calculate left ventricular mass index (LVMI). Then analyzed the correlation between smoking and the parathyroid hormone and LVMI. Results iPTH in the smoking group was higher than that in the no smoking group, and iPTH in the heavy smoking group was higher than that in the light smoking group either. Smoking was positively independent correlated with iPTH which increased with increasing amount of smoking. iPTH in the group that patients with left ventricular hypertrophy was higher than that without left ventricular hypertrophy, and iPTH had a positive correlation with left ventricular hypertrophy. Conclusion The increasing of Blood iPTH index is the key factor much contributing to left ventricular hypertrophy for end-stage renal disease patients who were smoking, which is worth to be concerned.
