目的:通过三维超声测量正常不同年龄段学龄前儿童肾脏体积,初步确定其参考值范围。方法入选标准:(1)年龄1个月至6岁;(2)无肾脏疾病史,其他辅助检查证实无泌尿系统疾病;(3)所有儿童均为正常足月产。采用Philips公司的IU22三维超声诊断仪测量501例1个月至6岁(平均年龄:2.7岁,中位年龄:2.8岁)学龄前儿童双侧肾脏大小,观察肾脏形态,记录性别、年龄及超声测量数据(肾脏长径、宽径及肾脏体积)。结果501例儿童中,467例双侧肾脏体积显示清晰,显示率为93.21%。(1)多元线性回归分析显示年龄与学龄前儿童肾脏体积有关(左侧:t=31.808,P<0.001;右侧:t=31.869, P<0.001);(2)直线相关(Spearman)分析显示年龄与学龄前儿童肾脏体积呈高度线性正相关(左侧:r=0.831,P<0.001;右侧:r=0.832,P<0.001);(3)配对样本t检验分析显示学龄前儿童肾脏体积左侧大于右侧(t=13.989,P<0.001);(4)单样本k-s检验分析学龄前儿童肾脏体积测量值服从正态分布(左侧:Z=0.157,P>0.05;右侧:Z=0.139,P>0.05),用均数±标准差( x ± s )确定参考值范围。结论学龄前儿童肾脏体积随着年龄增长而增大,本研究初步得出正常不同年龄段学龄前儿童双侧肾脏体积参考值范围,超出此范围之外应视为异常。

Objective To measure the renal volumes in healthy preschool children with different ages using three-dimensional ultrasonography. Methods A total of 501 healthy preschool children were normally born and without any clinical or laboratory evidence for diseases of urinary system.Their ages ranged from 1 month to 6 years old(mean age: 2.7 years, median age: 2.8 years). The renal volumes of them were determined using IU22 three-dimensional ultrasonography diagnostic apparatus. Results Among the 501 children, the renal volumes of 467 were clearly detected and measured(93. 21%). (1) Multiple linear regression analysis revealed the age was the risk factor associated with the bilateral renal volumes (left:t=31.808, P<0.001;right:t=31.869, P<0.001). (2) Spearman correlation analysis revealed that the renal volume was highly linear positively correlated with age (left:r=0.831, P<0.001; right: r=0.832, P<0.001). (3) Paired sample t test analysis revealed the left renal volume was greater than the right in preschool children (t=13.989, P<0.001). (4) Single sample k-s test analysis revealed the renal volume was normally distributed in preschool children(left: Z=0.157, P>0.05; right: Z=0.139, P>0.05). Conclusion The renal volume increased with ages in preschool children.This study preliminarily obtained reference range of bilateralrenal volumes at different ages of healthy preschool children.
