目的:探讨宝石能谱CT在小儿肺静脉异位引流中应用价值。方法回顾分析16例手术证实肺静脉异位引流患儿资料,所有患儿均使用低计量造影剂0.8~1.0 ml/kg,常规扫描后图像进入能谱界面处理,找出最佳单能图像,并行多平面重组(MPR)、薄层最大密度投影(MIP)和容积重建(VR)等后处理,并与彩超进行对照研究。结果所有患儿宝石能谱CT均显示肺静脉回流异常,利用宝石能谱最佳单能图像重建图像,16例均完整显示引流静脉行径,16例患儿引流位置诊断明确,显示率均高于彩超。重建图像能直观明确显示引流静脉的整体形态。结论小儿肺静脉异位引流利用宝石能谱CT检查,使用低剂量造影剂,用50~55 Kev单能图像重建,能很好的显示肺静脉异位引流细节,减少造影剂对小儿肾功能损害。

목적:탐토보석능보CT재소인폐정맥이위인류중응용개치。방법회고분석16례수술증실폐정맥이위인류환인자료,소유환인균사용저계량조영제0.8~1.0 ml/kg,상규소묘후도상진입능보계면처리,조출최가단능도상,병행다평면중조(MPR)、박층최대밀도투영(MIP)화용적중건(VR)등후처리,병여채초진행대조연구。결과소유환인보석능보CT균현시폐정맥회류이상,이용보석능보최가단능도상중건도상,16례균완정현시인류정맥행경,16례환인인류위치진단명학,현시솔균고우채초。중건도상능직관명학현시인류정맥적정체형태。결론소인폐정맥이위인류이용보석능보CT검사,사용저제량조영제,용50~55 Kev단능도상중건,능흔호적현시폐정맥이위인류세절,감소조영제대소인신공능손해。
Objective To explore the of application value precious stones energy spectrum CT in children with pulmonary vein ectopic drainage. Methods Retrospective analysis of 16 cases of confirmed pulmonary vein ectopic drainage operating data, low measuring contrast agents should be used in all children with 0.8 to 1.0 ml/kg, routine scan image spectrum into the interface processing, can find out the best single image, the parallel multi planar reconstruction (MPR), thin layer maximum density projection (MIP) and volume reconstruction (VR), and other post-processing, and compared with colour to exceed control study. Results All children with stones energy spectrum CT showed abnormal pulmonary vein reflux, energy spectrum can be the best single image reconstruction images using gems, 16 cases were showed venous drainage, drainage position clear diagnosis, 16 cases showed rates were higher than in colour to exceed. Reconstruction image can show the overall form of venous drainage clear intuitive. Conclusion Pediatric pulmonary vein ectopic drainage use of precious stones energy spectrum CT examination, use of low dose contrast agents, with 50 to 55 kev single image reconstruction, can display the pulmonary vein ectopic drainage good details, reduce the contrast of pediatric renal damage.
