COPD%TNF-α%IL-8%Changes
目的 探讨COPD患者急性发作期血清肿瘤坏死因子(TNF-α)和白细胞介素-8(IL-8)水平变化的意义.方法 回顾性分析2009至2012年我院收治的80例急性发作期的COPD患者,分为对照组和实验组.对照组为非呼吸衰竭组,实验组为呼吸衰竭组.采用放射免疫分析法(RIA)测定其血清TNF-α和IL-8水平进行比较,并检测其呼吸功能,探究其中的相关性.结果 实验组TNF-α和IL-8的血清含量均高于对照组,差异有显著性(P<0.05).实验组FEV1%小于对照组,差异有显著性(P<0.05).TNF-α和IL-8水平和患者的FEV1%呈负相关,相关系数分别为-0.72和-0.65(P< 0.05).结论 合并呼吸衰竭的COPD急性加重期患者同未合并呼吸衰竭的患者相比血清TNF-α和IL-8较高,TNF-α和IL-8的升高可能加重了COPD患者的气道阻塞,TNF-α和IL-8可以协助对患者的病情和预后进行监测.

Objective To explore the significance of changes in levels of TNF-α and IL-8 in acute exacerbation of COPD.Methods The data on 80 patients with acute exacerbation of COPD who had been hospitalized during the period of 2009 to 2011 were retrospectively analyzed.The patients were divided into two groups.The control group included 40 patients with respiratory failure; and the study group included 40 patients without respiratory failure.RIA was used to measure serum levels of TNF-α and IL-8; respiratory function was detected.The association of serum levels of TNF-α and IL-8 with respiratory function was determined.Results Levels of TNF-α and IL-8 were significantly higher in the study group than in the control group (P<0.05).FEV1% was significantly lower in the study group than in the control group (P<0.05).Levels of TNF-α and IL-8 had a negative correlation with FEV1%,with a correlation coefficient of-0.72 and-0.65 (P<0.05).Conclusions Patients with acute exacerbation of COPD complicated by respiratory failure have higher serum levels of TNF-α and IL-8 than those without respiratory failure.Increased levels of TNF-α and IL-8 may aggravate airway obstruction in COPD patients.TNF-α and IL-8 can help monitor disease severity and prognosis.
