目的 探讨责任护士临床思维模式的培养.方法 选择泌尿系疾病诊疗中心的20名责任护士为研究对象,2010年改革临床护理工作模式,护士与医生共同分管病人,制定知识、能力、素质“三位一体”层级培训计划,在日常工作实践中对责任护士观察、分析、判断和处理问题的综合素质能力进行持续培训,并对培训效果进行评价.结果 实施层级培训前后,20名责任护士专科知识、专科技能掌握程度和处理问题能力比较差异有统计学意义,护理文书中所反映专科护理内涵、安全护理等4项比较,差异有统计学意义.反映病情转归、治疗护理过程比较,没有统计学差异.结论 采用层级培训方式,运用护理程序方法,使责任护士从护理病人中获取专业知识和思维经验,是培养责任护士临床思维的最佳模式.

Objective To explore the training of clinical thinking mode of responsibility nurses.Methods Twenty responsibility nurses of the urologic disease treatment center were selected.The pattern of clinical nursing was reformed in 2010.The doctors and nurses managed patients together.The layer training plan was made which was knowledge,capacity,quality of the " trinity" according to requirements of layer management responsibility.The responsibility nurses were trained abidingly with the comprehensive quality including observing,analyzing,judging and dealing with the problem.The training effects were evaluated.Results Their special knowledge and mastery of special skills and ability of solving problem were siginificantly improved in 20 responsibility nurses which were performed layer training in nursing practice.The responsibility nurses after layer training were siginificant difference compared with before layer training (P < 0.01).The special nursing,secure nursing of nursing after layer training were siginificant difference compared with that before layer training (P <0.05).Disease prognosis,the process of treatment and nursing after layer training was no siginificant difference (P > 0.05).Conclusions In daily nursing practice,responsibility nurses can get expertise and thinking of experience from tending patients with layer training.It is the best mode of training clinical thinking of responsibility nurses
