目的:比较磁共振弥散加权成像(DWI)和动态增强(DCE)成像在子宫内膜癌分期中的价值。方法:分析58例子宫内膜癌患者术前磁共振成像,包括T1和T2加权成像、DWI(b=0和700 s/mm2)和DCE成像,比较平扫结合磁共振DWI、平扫结合磁共振DCE成像判断肌层浸润、总体分期的差异。结果:对于肌层浸润的评估,磁共振DCE成像整体准确率为77.6%(45/58),磁共振DWI整体准确率为91.4%(53/58)。磁共振DWI分期判断准确率高于DCE成像(χ2配对=6.356,P=0.015)。和病理结果相比,磁共振DWI Kappa为0.807(P<0.001),磁共振DCE成像为0.587(P<0.001)。结论:磁共振DWI与DCE成像相比,在子宫内膜癌肌层浸润和分期方面有更高的诊断准确率。

Aim:To compare the value of diffusion weighted imaging (DWI) and dynamic contrast enhanced (DCE) imaging in the staging of endometrial cancer .Methods:A total of 58 patients with endometrial cancer were collected .The preoperative magnetic resonance imaging including T 1 and T2 weighted imaging , DWI ( b=0 and 700 s/mm2 ) and DCE im-aging were analyzed .The difference between DWI , DCE imaging diagnosis of overall staging and myometrial invasion was observed.Results:For the assessment of myometrial invasion , DCE imaging overall accuracy was 77.6%(45/58), DWI overall accuracy was 91.4%(53/58) (χ2paired =6.356,P=0.015).Compared with pathological results , Kappa value for DWI was 0.807(P<0.001) and that for DCE imaging was 0.587(P<0.001).Conclusion:In myometrial invasion and staging, DWI has higher accuracy than DCE imaging .
