目的:探讨左心室舒张功能衰竭(DHF)患者外周血NF-κB、血清TGF-β1的水平变化及其与心室重构的相关性。方法选取150例DHF患者(实验组)和30例正常人(对照组),采用双抗体夹心酶联免疫吸附法(ELISA)检测外周血 NF-κB 和血清 TGF-β1的表达水平。利用心脏超声检测结果计算左心室心肌质量指数(LVMI)、平均室壁应力(MWS)。结果 DHF患者NF-κB、TGF-β1水平和LVMI、MWS均显著高于对照组(P<0.01),且随着心衰程度的加重而增加(P<0.01);DHF患者NF-κB、TGF-β1水平与反应心室重构的指标 LVMI、MWS 正相关(P<0.01)。多元线性回归分析显示,NF-κB、TGF-β1水平与LVMI均呈独立正相关(P<0.01)。结论 DHF患者NF-κB、TGF-β1水平增加,与DHF患者心室重构严重程度呈独立、显著的相关性。

Objective To investigate the expression changes of peripheral blood NF-κB and serum TGF-β1 in patients with left ventricular diastolic dysfunction(DHF) and explore their correlation with ventricular remodeling. Methods 150 patients diagnosed with DHF were recruited as the experimental group and 30 normal subjects were selected as control group. A double-antibody sandwich enzyme-linked immunosorbent assay(ELISA) was used to detect the peripheral blood NF-κB level and serum TGF-β1 level. Echocardiography was employed. Furthermore, left ventricular mass index (LVMI) and left ventricular mean wall stress (MWS) were calculated. Results Compared with the control group, the NF-κB levels, TGF-β1 levels, LVMI and MWS were significantly higher in DHF patients(P<0.01), and the NF-κB levels, TGF-β1 levels, LVMI and MWS rose correspondingly when cardiac function worsened(P<0.01). The NF-κB and TGF-β1 levels in DHF patients were positively correlated with LVMI and MWS(P<0.01). The positive association between NF-κB and LVMI, TGF-β1 and LVMI remained significant in a multivariate regression model(P<0.01). Conclusion Higher levels of peripheral blood NF-κB and serum TGF-β1 in DHF patients are significantly correlated with the severity of ventricular remodeling.
