目的:评价加长型气管切开导管在气管食管瘘中的作用。方法:10例气管食管瘘接受机械通气的患者,置入自制加长型气管切开导管,观察换管前、换管后1h、24h的呼吸力学、氧合指数与循环变化。结果:换管后1h、换管后24h组气道阻力较换管前组下降,氧合指数升高。换管后24h组平均动脉压较换管前组降低。结论:气管食管瘘患者采用加长型气管切开导管,可以改善机械通气时的呼吸力学和氧合。

목적:평개가장형기관절개도관재기관식관루중적작용。방법:10례기관식관루접수궤계통기적환자,치입자제가장형기관절개도관,관찰환관전、환관후1h、24h적호흡역학、양합지수여순배변화。결과:환관후1h、환관후24h조기도조력교환관전조하강,양합지수승고。환관후24h조평균동맥압교환관전조강저。결론:기관식관루환자채용가장형기관절개도관,가이개선궤계통기시적호흡역학화양합。
Objective:To evaluate the effects of lengthened tracheotomy tube for the treatment of tracheoesophageal fistula (TEF). Methods:A self-paired trial was conducted in 10 patients with TEF. Respiratory mechanics, PaO2/FiO2 and hemodynamics before and after lengthened tracheotomy tube placement were measured and compaired.Result:The airway resistance at 1h and 24h after lengthened tracheotomy tube placement was significantly lower than before, and the PaO2/FiO2 was higher. The MAP at 24h after lengthened tracheotomy tube placement was significantly lower than before. Conclusion:The lengthened tracheotomy tube improved the respiratory mechanics and PaO2/FiO2 in TEF with mechanical y ventilation.
