目的：比较乳腺癌保乳手术中应用弧形辅助放射状皮肤切口联合腺体瓣转移技术与传统梭形皮肤切口联合腺体瓣技术的疗效。方法62例符合保乳指征并要求行保乳治疗的乳腺癌患者，其中28例患者（实验组）在术中应用弧形辅助放射状皮肤切口联合腺体瓣转移技术，34例患者（对照组）常规行梭形皮肤切口，对其术后局部复发、远处转移、手术并发症、3年生存率、乳房美容效果满意度进行对比研究。结果术后乳房美容效果满意度调查实验组优于对照组（P＜0．05），而两组患者术后并发症、局部复发、远处转移、3年生存率相比较均无统计学差异（P＞0．05）。结论弧形辅助放射状皮肤切口联合腺体瓣转移技术符合乳腺癌手术治疗的原则，避免了过度切除皮肤并可低张力力学美容缝合皮肤，使乳房最大限度的恢复原貌。
목적：비교유선암보유수술중응용호형보조방사상피부절구연합선체판전이기술여전통사형피부절구연합선체판기술적료효。방법62례부합보유지정병요구행보유치료적유선암환자，기중28례환자（실험조）재술중응용호형보조방사상피부절구연합선체판전이기술，34례환자（대조조）상규행사형피부절구，대기술후국부복발、원처전이、수술병발증、3년생존솔、유방미용효과만의도진행대비연구。결과술후유방미용효과만의도조사실험조우우대조조（P＜0．05），이량조환자술후병발증、국부복발、원처전이、3년생존솔상비교균무통계학차이（P＞0．05）。결론호형보조방사상피부절구연합선체판전이기술부합유선암수술치료적원칙，피면료과도절제피부병가저장력역학미용봉합피부，사유방최대한도적회복원모。
Objective To compare the efficacy of the application of arc assisted radical incision joint glandular flap transfer technique in breast-conserving surgery for breast cancer compared with the conventional fusiform incision joint glandular flap transfer technique .Methods 62 breast cancer patients with the breast-conserving indications required breast-conserving sur-gery.Arc assisted radical incision joint glandular flap transfer technique was applied to 28 patients in the experimental group and the conventional fusiform incision was applied to 34 patients in the control group.Postoperative local relapse,distant metastasis, complications,3-year survival rate and satisfactory of cosmetic outcomes were investigated and compared between the 2 groups. Results Satisfactory of breast cosmetic effects of the experimental group was better than that of the control group (P<0.05), postoperative local relapse ,distant metastasis ,complications and 3-year survival rate of the 2 groups had no statistical difference (P>0.05).Conclusion Arc assisted radical incision joint glandular flap transfer technique ,conforms to the principle of breast cancer therapy ,it can avoide excessive resection and restore breast at utmost .
