目的:探讨头颈部异位甲状腺的分布规律及CT诊断价值,以提高诊断水平。方法回顾性分析12例证实的头颈部异位甲状腺患者的临床及影像学资料,比较超声、CT及核素的诊断优势。结果本组头颈部异位病灶主要分布于甲状腺水平及以上部位的口腔和颈部,其中颅底岩尖处1例,口腔舌根2例,颈前外侧区3例,口腔舌根-颈前外区2例,气管内2例,气管旁2例。3例有2个异位病灶。单纯异位甲状腺3例共4个病灶,异位结节性甲状腺肿9例共11个病灶。常见临床表现有吞咽不适、呼吸困难、颈部包块。本组病例的病灶检出率CT高于超声及核素。结论头颈部异位甲状腺分布具有一定规律,CT对发现及诊断ETG具有重要优势,可发现超声难以发现的特殊部位的病灶,能提供清晰直观的解剖关系,为临床处理提供重要依据。

목적:탐토두경부이위갑상선적분포규률급CT진단개치,이제고진단수평。방법회고성분석12예증실적두경부이위갑상선환자적림상급영상학자료,비교초성、CT급핵소적진단우세。결과본조두경부이위병조주요분포우갑상선수평급이상부위적구강화경부,기중로저암첨처1례,구강설근2례,경전외측구3례,구강설근-경전외구2례,기관내2례,기관방2례。3례유2개이위병조。단순이위갑상선3례공4개병조,이위결절성갑상선종9례공11개병조。상견림상표현유탄인불괄、호흡곤난、경부포괴。본조병례적병조검출솔CT고우초성급핵소。결론두경부이위갑상선분포구유일정규률,CT대발현급진단ETG구유중요우세,가발현초성난이발현적특수부위적병조,능제공청석직관적해부관계,위림상처리제공중요의거。
Objective To study the distribution and value of CT in the diagnosis of ectopic thyroid gland in the head and neck. Methods The clinical,ultrasound,CT and radionuclide findings of 12 patients with histologically confirmed ectopic thyroid glands in the head and neck were retrospectively analyzed.Results All of the ectopic thyroid glands were distributed in the oral cavity and neck including the petrous apex(1),base of tongue(2),anterolateral neck(3),base of tongue-anterolateral neck(2),intra-tracheal(2)and lateral to the trachea(2).Three patients had two ectopic lesions.There were 3 patients with 4 normal-appearing ectopic thyroid glands and 9 patients with 11 ectopic nodular goiters.Clinical symptoms included discomfort of deglutition,dyspnea and cervical mass. Detection rate of CT was higher than both ultrasound and radionuclide studies.Conclusions CT can clearly show the ectopic thyroid gland and the surrounding anatomy.
