目的:通过对从化地区上尿路结石的成分分析,以及对草酸钙结石患者进行饮食危险因素的病例对照研究,探讨该地区尿路结石形成的病因学,为临床预防、治疗提供依据。方法回顾性分析167例上尿路结石患者的临床资料,对结石标本进行化学成分测定,对确诊为草酸钙结石的患者进行病例对照研究,收集患者饮食危险因素。结果上尿路结石男性发病率高于女性,男女比例为2.63:1,结石中含草酸钙检出率最高,混合结石几乎均含有草酸钙成分;经单因素条件logistic回归分析,喜饮酒、喜食蔬菜、喜食海鲜、喜煲汤、喜腌制菜等因素与草酸钙结石的发病差异有统计学意义;多因素条件Logistic回归显示,喜饮酒、喜食海鲜、喜煲汤及喜腌制菜的回归系数都为正值,OR值>l,差异有统计学意义。结论结石成分分析及结石饮食危险因素的病例对照研究对尿路结石的形成及预防具有重要意义。

목적:통과대종화지구상뇨로결석적성분분석,이급대초산개결석환자진행음식위험인소적병례대조연구,탐토해지구뇨로결석형성적병인학,위림상예방、치료제공의거。방법회고성분석167례상뇨로결석환자적림상자료,대결석표본진행화학성분측정,대학진위초산개결석적환자진행병례대조연구,수집환자음식위험인소。결과상뇨로결석남성발병솔고우녀성,남녀비례위2.63:1,결석중함초산개검출솔최고,혼합결석궤호균함유초산개성분;경단인소조건logistic회귀분석,희음주、희식소채、희식해선、희보탕、희업제채등인소여초산개결석적발병차이유통계학의의;다인소조건Logistic회귀현시,희음주、희식해선、희보탕급희업제채적회귀계수도위정치,OR치>l,차이유통계학의의。결론결석성분분석급결석음식위험인소적병례대조연구대뇨로결석적형성급예방구유중요의의。
ObjectiveTo investigate the etiology of formation of urinary tract stones in conghua area,the analysis of chemical constituents was carried out for 167 cases upper urinary tract stones.and the patients with calcium oxalate stones were conducted by case-control study of dietary risk factor.which can provide the basis for prevention and treatment of urinary tract.MethodsCinical date of stone patients was analyzed periods,and stones samples was carried by chemical analysis;The dietary risk factors of patients were collected for case-control study.ResultsUpper urinary stones were more often found in males than in females,and the ratio was 2.63:1.The most common component of the urinary stones are calcium oxalate stones,mixed stonescontains almost composition of calcium oxalate;single-factor conditional logistic regression displayed that drinking,vegetables,seafood,puree,sauerkraut and the occurrence of calcium oxalate have statistic differences.Multivariate conditional logistic regression showed that the regression coefficient of tack of drinking,vegetables,seafood,puree,sauerkraut are positive.which have statistic differences similarly. ConclusionChemical composition analysis and case-control study may provide important information to the etiology and prevention of urinary stones.
