目的:探讨Claudin-1和MMP-2蛋白在视网膜母细胞瘤中的表达变化及其与视网膜母细胞瘤组织分化、视神经浸润和临床分期的相关性。方法采用免疫组化方法(MaxVisionTM)检测Claudin-1、MMP-2蛋白在45例视网膜母细胞瘤和15例正常视网膜组织石蜡标本的表达,运用卡方检验和Spearman等级相关检验分析Clandin-1和MMP-2在视网膜母细胞瘤组织中表达的相关性。结果(1)Claudin-1蛋白在视网膜母细胞瘤组织阳性表达明显低于在正常视网膜组织;在分化型组阳性表达显著高于未分化型组,P=0.015;在未侵犯视神经组阳性表达明显高于侵犯视神经组,P<0.001;在临床Ⅰ期、Ⅱ期、Ⅲ期组中各组间表达均具有统计学差异,P<0.01;不同性别组中Claudin-1表达没有统计学差异,P=0.661。(2)MMP-2蛋白在视网膜母细胞瘤中阳性表达明显高于正常视网膜组织细胞;在分化型组阳性表达中表达低于未分化型组,表达没有统计学差异,P=0.636;在侵犯视神经组阳性表达明显高于未侵犯视神经组,P=0.011;在临床Ⅰ期组、Ⅱ期和Ⅲ期各期之间中表达均具有统计学差异,P<0.05;在不同性别组中表达没有统计学差异,P=0.58。(3)在视网膜母细胞瘤中Claudin-1表达下降和MMP-2表达上升两者呈负相关(r=-0.537,P=0.023)。结论 Claudin-1表达水平与视网膜母细胞瘤细胞分化、视神经浸润和临床分期呈正相关;MMP-2表达水平与视网膜母细胞瘤视神经浸润和临床分期呈负相关。Clandin-1和MMP-2在视网膜母细胞瘤的视神经浸润与肿瘤发展起相反作用。

Objective To study the protein expression of Claudin-1 and MMP-2 protein in retinoblastoma and their correla-tion with retinoblastoma tissue differentiation,clinical optic nerve infiltration capacity and retinoblastoma staging. Methods Im-munohistochemistry was used to analyze the expression of Claudin-1 and MMP-2 in 45 cases of retinoblastoma and 15 cases of normal retina tissue specimens. Chi-square test and Spearman rank correlation analysis was used to study correlation between the expression of Clandin-1 and MMP-2 in retinoblastoma tissues. Results (1) There was significant difference of the expression of Claudin-1 in retinoblastoma and normal retina,P<0.001;the positive rate of Claudin-1 expression in clinical stageⅠ,Ⅱ,Ⅲgroup was statistically significant different,P<0.01;The expression of Claudin-1 in differentiated retinoblastoma was significantly higher than that in undifferentiated group,P=0.015;Its expression in the retinoblastoma group without optic nerve invasion group was sig-nificantly higher than that in the group with optic nerve invasion,P<0.001;There was no significant difference in male and femal, P=0.661. (2)There was significant difference of the expression of MMP-2 in retinoblastoma and normal retina ,P<0.01;the positive rate of MMP-2 expression in clinical stage Ⅰ,Ⅱ,III group was statistically significant different,P<0.05. The expression of MMP-2 in differentiated retinoblastoma was no significant different than that in undifferentiated group ,P=0.636;Its expression in the retinoblastoma group without optic nerve invasion group was significantly lower than that in the group with optic nerve invasion ,P=0.011;There was no significant difference in male and femal,P=0.586. (3)Claudin-1 and MMP-2 had a negative correlation in retinoblastoma (r=-0.537,P=0.023). Conclusion The expression level of Claudin-1 in retinoblastoma had close relation with retinoblastoma cellular differentiation,optic nerve invasion and clinical stage. The lower Claudin-1 experssed,the lower in retinoblastoma cell differentiated,which could could promote retinoblastoma tumor invasion and development. MMP-2 expression level was correlated with optic nerve invasion and clinical staging of retinoblastoma The higher MMP-2 experseesd ,the more ag-gressive the retinoblastoma tumor was. Clandin-1 and MMP-2 effected oppositely in the optic nerve invasion and tumor develop-ment of retinoblastoma.
