目的：介绍“围巾式”食管-胃吻合术预防术后吻合口瘘的临床经验。方法回顾性分析2002年1月至2014年1月对75例门静脉高压症并食管下段静脉曲张出血病例行食管下段及胃底切除术时使用“围巾式”食管-胃吻合术治疗的结果。观察术后并发症发生率和病死率。结果75例手术无吻合口瘘发生，吻合口狭窄3例，术后复发出血2例，手术死亡1例。结论“围巾式”食管-胃吻合术可减少食管下段及胃底切除术后吻合口并发症，安全、有效。
목적：개소“위건식”식관-위문합술예방술후문합구루적림상경험。방법회고성분석2002년1월지2014년1월대75례문정맥고압증병식관하단정맥곡장출혈병례행식관하단급위저절제술시사용“위건식”식관-위문합술치료적결과。관찰술후병발증발생솔화병사솔。결과75례수술무문합구루발생，문합구협착3례，술후복발출혈2례，수술사망1례。결론“위건식”식관-위문합술가감소식관하단급위저절제술후문합구병발증，안전、유효。
Objective To summarize institutional experiences of scarf-like gastroesophageal anas-tomosis for preventing anastomotic leak.Methods Analyses were performed for the clinical outcomes of 75 consecutive patients undergoing scarf-liking gastroesophageal anastomosis after lower esophagus-proximal gastric resection in portal hypertension with lower esophagus variceal bleeding from Janu-ary 2002 to January 2004.Results No anastomotic leak was found.Anastomotic stricture occurred in 3 patients,another 2 had postoperative re-bleeding and one died.Conclusions The novel scarf-like gas-troesophageal anastomosis is both safe and effective and it may reduce anastomotic complications after lower esophagus-proximal gastric resection.
