目的:探讨螺旋CT三维重建技术在下颌骨畸形整形中的应用价值。方法:自2012年3月-2013年3月我院收治的下颌骨畸形患者18例,对其术前术后进行螺旋CT断层扫描和三维重建,垂直测量咬合平下颌面部位第一磨牙、第二前磨牙、第二磨牙至下颌骨下缘的距离和下颌骨升支中部至下颌角的距离,并正常侧分别与术前畸形侧、术后畸形侧上述距离的比较。结果:正常侧和术前畸形侧的第一磨牙、第二前磨牙、第二磨牙至下颌骨下缘的垂直距离和下颌骨升支高度进行对比,差异显著,有统计学意义(P<0.05)。正常侧与术后畸形侧比较上述距离差异无统计学意义。结论:下颌骨畸形整形中运用螺旋CT三维重建技术,可以准确判断手术部位,精确测量截骨量,保证手术安全、顺利进行,具有极大的应用价值。

목적:탐토라선CT삼유중건기술재하합골기형정형중적응용개치。방법:자2012년3월-2013년3월아원수치적하합골기형환자18례,대기술전술후진행라선CT단층소묘화삼유중건,수직측량교합평하합면부위제일마아、제이전마아、제이마아지하합골하연적거리화하합골승지중부지하합각적거리,병정상측분별여술전기형측、술후기형측상술거리적비교。결과:정상측화술전기형측적제일마아、제이전마아、제이마아지하합골하연적수직거리화하합골승지고도진행대비,차이현저,유통계학의의(P<0.05)。정상측여술후기형측비교상술거리차이무통계학의의。결론:하합골기형정형중운용라선CT삼유중건기술,가이준학판단수술부위,정학측량절골량,보증수술안전、순리진행,구유겁대적응용개치。
Objective:To study the application value of spiral CT three-dimensional reconstruction technology in plastic mandibular malformation. Method:From March,2012 to March,2013,18 cases of mandibular malformation patients admitted in March,the spiral CT scanning and three-dimensional reconstruction, vertical measurement was done on maxillofacial first molars,the second premolars,the distance to the edge of mandibular second molars and the distance of mandibular central branch to mandibular angle,and the distance among the normal,malformation before operation and malformation after operation were compared. Result:Maxillofacial first molars,the second premolars,the distance to the edge of mandibular second molars and the distance of mandibular central branch to mandibular angle of the normal side and malformation before operation was compared,with significant difference and with statistical significance(P<0.05). There was no statistical meaning between normal side and malformation side after operation. Conclusion:Mandibular deformities in the plastic using spiral CT 3 d reconstruction technology can accurately judge the surgical site,accurate measurement osteotomy,to assure the safety of surgery and smoothly,and it has great application value.
