目的:通过分析血管性痴呆肾虚证与非肾虚证患者间的差异表达蛋白,为脑梗死后血管性痴呆中医辨证分型提供可行的客观依据。方法采用表面增强激光解吸离子化飞行时间质谱(SELDI-TOF-MS)技术及PBSⅡc型蛋白质芯片,检测47例血管性痴呆肾虚证与非肾虚证患者以及13例健康者的血清蛋白指纹图谱。对相同质荷比的差异表达蛋白峰值做线性分类分析,经过进一步优化试验参数,初步确定血管性痴呆不同证型指纹图谱数据库。结果在相对分子质量1000~50000范围内,脑梗死后发生血管性痴呆患者与健康者中共检出25个差异表达蛋白质峰(P<0.05),血管性痴呆肾虚证与非肾虚证患者中共检出15个差异表达蛋白质峰,差异有统计学意义(P<0.05)。结论SELDI-TOF-MS结合蛋白质芯片技术能够快速初步筛选出血管性痴呆肾虚证与非肾虚证患者血清中的特异表达标志物。

Objective To supply objective evidence of syndrome differentiation about vascular dementia after stroke by detecting the protein expression in patients suffered with vascular dementia and observing the differences between the patients with or without kidney deficiency .Methods A total of 47 patients ,including 30 cases with kid-ney deficiency and 17 cases without kidney deficiency ,and 13 cases of health subjects were enrolled .Serum protein fingerprints of patients and health subjects were detected by using surface-enhanced laser desorption time of flight mass spectrometry (SELDI-TOF-MS) and PBSⅡc protein chips .Classification analysis was performed to identify protein peak values which had the same mass-to-charge ratio .Then ,the test parameters were optimized ,and the data-base of fingerprints about different syndromes of vascular dementia patients was constructed .Results At the range of relative molecular mass from 1 000 to 50 000 ,25 variability protein peaks were found between patients and health subjects (P<0 .05) ,and 15 variability protein peaks were found between patients with or without kidney deficiency (P<0 .05) .Conclusion By using SELDI-TOF-MS and protein chips ,the differential markers in serum of vascular dementia patients with or without kidney deficiency could be quickly and preliminary screened .
