目的:探讨新生儿缺氧缺血性脑病(HIE)的血清生化,包括钾、钠、氯、钙和葡萄糖的变化及其临床意义。方法:对165例不同程度的HIE患儿(HIE组)进行血液中钾、钠、氯、钙和葡萄糖检测,并与35例正常新生儿(对照组)血液中上述指标进行比较。结果:HIE组与对照组血清电解质比较,血钠和血钙明显降低,血钾升高,差异有统计学意义(P<0.05);两组氯离子比较,差异无统计学意义(P>0.05);不同程度HIE中电解质钠和钙降低程度与HIE的分度有明显的正相关关系,相关系数分别为0.841(P<0.05)和0.654(P<0.05);轻中度HIE患儿以低血糖为主,随着HIE临床分度的增加,高血糖发生率增高。结论:HIE患儿可出现低钠、低钙和高钾血症及糖代谢紊乱,HIE程度越重,血钠、血钙水平越低,且彼此间呈正相关,而高血糖的发生率越高,提示血钠、血钙和血糖对判断HIE病情严重程度及预后具有一定的临床意义。

목적:탐토신생인결양결혈성뇌병(HIE)적혈청생화,포괄갑、납、록、개화포도당적변화급기림상의의。방법:대165례불동정도적HIE환인(HIE조)진행혈액중갑、납、록、개화포도당검측,병여35례정상신생인(대조조)혈액중상술지표진행비교。결과:HIE조여대조조혈청전해질비교,혈납화혈개명현강저,혈갑승고,차이유통계학의의(P<0.05);량조록리자비교,차이무통계학의의(P>0.05);불동정도HIE중전해질납화개강저정도여HIE적분도유명현적정상관관계,상관계수분별위0.841(P<0.05)화0.654(P<0.05);경중도HIE환인이저혈당위주,수착HIE림상분도적증가,고혈당발생솔증고。결론:HIE환인가출현저납、저개화고갑혈증급당대사문란,HIE정도월중,혈납、혈개수평월저,차피차간정정상관,이고혈당적발생솔월고,제시혈납、혈개화혈당대판단HIE병정엄중정도급예후구유일정적림상의의。
Objective:To explore the change and their clinical significance of K+,Na+,Cl-,Ca2+ and glucose in serum of hypoxia-ischemic encephalopathy(HIE).Method:Testing 165 serum samples from HIE and 35 serum samples from normal neonatal for K+,Na+,Cl-,Ca2+,glucose in order to compare with each other.Result:Na+and Ca2+in serum decrease obviously and K+increased in HIE group,the difference between two groups was statistically significant(P<0.05);But the Cl-level had no significant change(P>0.05).Different degree of HIE and the decrease of Na+and Ca2+had significantly positive correlation and r=0.841(P<0.05),0.654(P<0.05).Hypoglycemia much more happened in mild and moderate HIE,the incidence of hyperglycemia increased with the clinic degree of HIE.Conclusion:Children with HIE can appear low level of Ca2+,Na+and high level of K+and glucose metabolic disorder.The more serious of HIE degree,the lower are Ca2+,Na+in blood,which are significantly positive correlation with increased incidence of hyperglycemia.Above results indicate that the K+,Na+,Ca2+and glucose in blood have certain clinical significance to justice the degree of HIE and its prognosis.
