目的:探讨早产儿脑白质损伤(cerebral white matter damage,CWMD)的早期 MRI表现。方法选取我院2011年7月~2012年10月临床及 MRI诊断CWMD 37例。结果37例早期CWMD 1 w内弥散加权成像(dif usion weighted imaging,DWI)呈高信号,局灶性损伤32例,T1等或稍高信号,T2等或稍低信号,弥漫性损伤5例,T1等或稍高信号,T2稍高信号。病灶位于侧脑室三角区和枕角周围脑白质23例,侧脑室前角周围脑白质16例,侧脑室体部周围脑白质19例。结论早期CWMD临床表现缺乏特异性,DWI能发现常规MRI难以显示的早期病变,对早期脑白质损伤的敏感性和特异性非常高。

목적:탐토조산인뇌백질손상(cerebral white matter damage,CWMD)적조기 MRI표현。방법선취아원2011년7월~2012년10월림상급 MRI진단CWMD 37례。결과37례조기CWMD 1 w내미산가권성상(dif usion weighted imaging,DWI)정고신호,국조성손상32례,T1등혹초고신호,T2등혹초저신호,미만성손상5례,T1등혹초고신호,T2초고신호。병조위우측뇌실삼각구화침각주위뇌백질23례,측뇌실전각주위뇌백질16례,측뇌실체부주위뇌백질19례。결론조기CWMD림상표현결핍특이성,DWI능발현상규MRI난이현시적조기병변,대조기뇌백질손상적민감성화특이성비상고。
Objective:To study the MRI (magnetic resonance imaging) characteristics of the preterm infants with early cerebral white mat er damage (CWMD). Methods:The research was prospectively performed on 37preterm infants were diagnosed CWMD according to MRI diagnostic criteria from July 2011 to October 2012 of our Hospital. Al were performed MRI examination in 7 days. Results:The lesions in early stage of CWMD,high signal appeared on DWI (dif usion weighted imaging) . 32 cases of focal,T1 equisignal or slightly high signal and T2 equisignal or slightly low signal,5 cases of dif use lesions ,T1 equisignal or slightly high signal and T2 slightly high signal. 23 cases Lesions located in the lateral ventricle triangle and occipital horn ,16 cases around the anterior horn of lateral ventricle,19 cases around the body of lateral ventricle. Conclusion:CWMD early clinical manifestations of the lack of specificity,DWI can find conventional MRI can display the early pathological changes,of sensitivity and specificity for early cerebral white mat er lesions are very high.
