目的：探讨弹性髓内钉内固定治疗儿童股骨干的临床疗效和优势。方法：回顾性分析2007年1月-2012年12月行弹性髓内钉内固定的21例5岁以上儿童股骨干骨折的临床资料及疗效。结果：本组21例均获随访，随访时间6~36个月，平均12个月。术后6周开始逐步进行负重训练，骨折均愈合，愈合时间8~24周。患肢髋、膝关节活动度正常，功能恢复良好，无跛行。出现皮肤激惹症状5例，无髓内钉断裂、弯曲发生；无再骨折及骨髓炎发生。弹性髓内钉术后8周所有骨折达到影像学愈合。平均术后22周取出弹性钉。结论：弹性髓内钉治疗5岁以上儿童股骨干骨折疗效明显，具有手术时间短、创伤小、愈后快以及并发症少等优点，是一种5岁以上儿童股骨干骨折较为理想的临床治疗方法。
목적：탐토탄성수내정내고정치료인동고골간적림상료효화우세。방법：회고성분석2007년1월-2012년12월행탄성수내정내고정적21례5세이상인동고골간골절적림상자료급료효。결과：본조21례균획수방，수방시간6~36개월，평균12개월。술후6주개시축보진행부중훈련，골절균유합，유합시간8~24주。환지관、슬관절활동도정상，공능회복량호，무파행。출현피부격야증상5례，무수내정단렬、만곡발생；무재골절급골수염발생。탄성수내정술후8주소유골절체도영상학유합。평균술후22주취출탄성정。결론：탄성수내정치료5세이상인동고골간골절료효명현，구유수술시간단、창상소、유후쾌이급병발증소등우점，시일충5세이상인동고골간골절교위이상적림상치료방법。
Objective:To evaluate the clinical effect and advantages of elastic intramedullary nail in treatment of femoral shaft in children. Method:Retrospectively analyzed the clinical data and curative effect from January 2007 to December 2012,21 cases of children femoral shaft fractures were treated by elastic intramedullary nail. Result:21 patients were followed up,the follow-up time was 6-36 months,mean 12 months;all patients started weight training after 6 weeks operation,All of the fractures healed ,the healing time about 8 weeks to 24 weeks,the children of the hip,knee joint returned to normal activity. The fracture side limb recovered good functional. 5 patients had skin irritation symptoms,without elastic intramedullary nail breakage and bending;no refracture and osteomyelitis. All fractures achieved radiographic union after 8 weeks operation,average elastic intramedullary nails were removed 22 weeks after operation. Conclusion:The elastic intramedullary nail in treatment of femoral shaft fracture in children has obvious curative effect,can shorten operation time,less trauma,healing and fewer complications.
