目的:探讨经后路手术治疗胸腰椎爆裂骨折中不同植骨融合术的疗效。方法将2009年1月-2012年1月我院60例胸腰椎爆裂骨折患者随机分为两组,治疗组行后路内固定椎体加椎间植骨融合术,对照组行内固定术后外侧植骨融合术,观察两组疗效。结果术后12个月时治疗组椎体Cobb角明显好于对照组(P <0.05);治疗组骨折愈合率及内固定失效率明显优于对照组(P <0.05);治疗组术后神经功能Frankel分级评分明显优于对照组(P <0.05)。结论椎体椎间植骨融合术结合后路复位内固定术治疗胸腰椎爆裂性骨折,能促进骨折愈合,减少内固定失效率,值得临床推广。

목적:탐토경후로수술치료흉요추폭렬골절중불동식골융합술적료효。방법장2009년1월-2012년1월아원60례흉요추폭렬골절환자수궤분위량조,치료조행후로내고정추체가추간식골융합술,대조조행내고정술후외측식골융합술,관찰량조료효。결과술후12개월시치료조추체Cobb각명현호우대조조(P <0.05);치료조골절유합솔급내고정실효솔명현우우대조조(P <0.05);치료조술후신경공능Frankel분급평분명현우우대조조(P <0.05)。결론추체추간식골융합술결합후로복위내고정술치료흉요추폭렬성골절,능촉진골절유합,감소내고정실효솔,치득림상추엄。
Objective To investigate clinical efficacy of two kinds of Interbody fusion surgery in the treatment of thoracic and lumbar burst fractures. Methods Selected 60 cases patients admitted to our hospital from January 2009 to January 2012 were divided into treatment group(given fixed vertebral interbody fusion with posterior) and control group(given outside of the internal fixation and fusion), the efficacy of two groups were compared. Results The vertebral cobb angle of two gropes had statistically significant after 12 months(P <0.05);The rate of fracture healing and fixation failure rate of treatment group were better than that of control group(P <0.05); The postoperative neurological Frankel grading system of treatment group was better than that of control group(P <0.05). Conclusion Vertebral interbody fusion combined with posterior reduction and internal fixation has a significant effect on treating thoracic and lumbar burst fractures, which is worthy of clinical widely.
