目的分析妊娠高血压疾病的血脂代谢特点，了解血脂代谢紊乱和妊娠高血压疾病的关系。方法选取孕周为37～41周的健康妊娠孕妇60例（正常妊娠组），妊娠高血压疾病孕妇90例（妊娠高血压疾病组），正常未孕女性60例（未孕组），比较3个组的三酰甘油（TG）、总胆固醇（TC）、高密度脂蛋白-胆固醇（ HDL-C）、低密度脂蛋白-胆固醇（LDL-C）、载脂蛋白-A（Apo-A）、载脂蛋白-B （Apo-B）及脂蛋白a（Lpa）之间的差异。同时将90例妊娠高血压疾病孕妇按病程分级分为妊娠高血压组30例、轻度子痫前期组30例及重度子痫前期组30例共3个组，并比较不同分级的妊娠高血压疾病孕妇血脂的变化，分析血脂异常程度与妊娠高血压疾病严重程度的关系。结果（1）未孕组、正常妊娠组及妊娠高血压疾病组间TC 、TG 、HDL-L 、LDL-L 、Apo-A 及Apo-B 水平差异有统计学意义（P＜0．05）；而3个组间Lpa 水平差异无统计学意义（P ＞0．05）。（2）在不同病程妊娠高血压疾病中，妊娠高血压组、轻度子痫前期组及重度子痫前期组3个组的TG 、LDL-C 、Apo-B 水平比较差异有统计学意义（ P ＜0．05），而 TC 、HDL-L 、Apo-A 及Lpa 各组之间差异无统计学意义（P ＞0．05）。结论妊娠高血压疾病的发生可能与孕期血脂代谢紊乱相关，血脂异常程度越高，发生严重妊娠高血压疾病的可能性就越大，故加强血脂监测，早期进行干预并纠正血脂紊乱以保证母婴健康。
목적분석임신고혈압질병적혈지대사특점，료해혈지대사문란화임신고혈압질병적관계。방법선취잉주위37～41주적건강임신잉부60례（정상임신조），임신고혈압질병잉부90례（임신고혈압질병조），정상미잉녀성60례（미잉조），비교3개조적삼선감유（TG）、총담고순（TC）、고밀도지단백-담고순（ HDL-C）、저밀도지단백-담고순（LDL-C）、재지단백-A（Apo-A）、재지단백-B （Apo-B）급지단백a（Lpa）지간적차이。동시장90례임신고혈압질병잉부안병정분급분위임신고혈압조30례、경도자간전기조30례급중도자간전기조30례공3개조，병비교불동분급적임신고혈압질병잉부혈지적변화，분석혈지이상정도여임신고혈압질병엄중정도적관계。결과（1）미잉조、정상임신조급임신고혈압질병조간TC 、TG 、HDL-L 、LDL-L 、Apo-A 급Apo-B 수평차이유통계학의의（P＜0．05）；이3개조간Lpa 수평차이무통계학의의（P ＞0．05）。（2）재불동병정임신고혈압질병중，임신고혈압조、경도자간전기조급중도자간전기조3개조적TG 、LDL-C 、Apo-B 수평비교차이유통계학의의（ P ＜0．05），이 TC 、HDL-L 、Apo-A 급Lpa 각조지간차이무통계학의의（P ＞0．05）。결론임신고혈압질병적발생가능여잉기혈지대사문란상관，혈지이상정도월고，발생엄중임신고혈압질병적가능성취월대，고가강혈지감측，조기진행간예병규정혈지문란이보증모영건강。
Objective To analyze the characteristics of lipid metabolism in pregnant women with hypertensive disorders .Methods A total of 60 cases of healthy pregnant women in the gestational age of 37-41 weeks (healthy pregnancy group) ,90 cases of pregnant women hypertensive disorders (hypertensive disorders group) and 60 healthy unpregnant women(unpregnant group) were enrolled ,and compared for blood levels of triglyceride (TG) ,total cho-lesterol(TC) ,high density lipoprotein-cholesterol(HDL-C) ,low density lipoprotein-cholesterol(LDL-C) ,apolipopro-tein-A(Apo-A) ,apolipoprotein-B(Apo-B) and lipoprotein a(Lpa) .The 90 cases of pregnant women with hypertensive disorders were divided into three groups ,including gestational hypertension group(30 cases) ,mild preeclampsia group (30 cases) ,severe preeclampsia group(30 cases) ,and levels of blood lipids were compared between each group to i-dentify the relationship between lipids and hypertensive disorders in pregnancy .Results (1)Levels of TC ,TG ,HDL-C ,LDL-C ,Apo-A and Apo-B were statistically different among healthy pregnancy group ,hypertensive disorders group and unpregnant group(P<0 .05) ,while Lpa levels were not statistically different (P> 0 .05) .(2) Levels of TG ,LDL-C and Apo-B were significantly different among gestational hypertension group ,mild preeclampsia group and severe preeclampsia group(P<0 .05) ,while levels of TC ,HDL-C ,Apo-A and Lpa were not significantly different (P> 0 .05) .Conclusion Hypertensive disorders in pregnancy might be correlated with pregnancy-related dyslipi-demia .The onset risk of hypertensive disorders in pregnancy could be increased with the elevation of the severity of dyslipidemia .Enhancing lipid monitoring ,early intervention of dyslipidemia could ensure maternal and child health .
