历代医家对“以痛为输”进行了解释，近代学者结合临床提出了不同的意见，主要集中于两个方面，一是对疼痛点的不同认识，二是与阿是穴的关系。重温《内经》有关条文，认为“以痛为输”是《灵枢·经筋》提出的取穴方法，而经筋病变多集中于入腠袭筋之体表筋肉之处，其症候主要表现为疼痛为主，并以压痛为主要病理反应，而经筋不能传输气血，无穴可言，唯有“以痛为输”作为治疗经筋痹证的优化取穴方法。“以痛为输”属阿是取穴法的一种，与经穴、奇穴以外无明确定位的一类腧穴的总称---阿是穴的概念不同，当明辨之。

Doctors throughout the ages have explanations on “pain-point needling”.Modern scholars com-bine clinic and put forward different opinions , mainly focus on two aspects , one is a different understanding of the pain points , and the other is the relationship with the Ashi points .Author reviews related provisions in“Neijing”, consider “pain -point needling” is the acupoint selection method raised out from “Lingshu Jingjin”, and meridian sinews diseases mostly concentrated on the location between the body surface and the muscles, where it's entering striae invading sinews , its main symptom is pain , main pathologic response is ten-derness .Meridian sinews cannot transmit energy and blood , so no acupoints can mention , only “pain-point needling” is the optimized acupoint selection method for treating meridian sinews Bi -syndrome .“Pain-point needling” is one of the Ashi point selection methods , it differs from Ashi point's concept ( a general name for a category of the acupoints without clear locations ) , other than meridian acupoints and extra -meridian acu-points ) ,which should be distinguished clearly .
