目的:探讨联合检测心脏型脂肪酸结合蛋白(H-FABP)、心肌肌钙蛋白I(cTnI)、脑钠肽(BNP)在急性心肌梗死(AMI)早期诊断中的意义。方法选择以急诊入院的患者116例,其中 AMI患者64例,非AMI患者52例。用胶体金方法检测患者发病0~3 h和3~12 h血清中 H-FABP ;用化学发光分析方法定量检测cTnI、BNP的浓度,用统计学方法分析 H-FABP、cTnI、BNP项目的敏感性和特异性。结果 AMI组发病0~3 h测定的 H-FABP、cTnI、BNP的敏感性分别是95.45%、59.09%、61.36%;发病3~12 h后测定的 H-FABP、cTnI、BNP的敏感性和特异性分别是97.73%、97.73%、86.36%和90.63%、84.38%、81.25%。结论发病0~3 h检测 H-FABP对AMI敏感性较高,具有早期诊断价值,在发病3~12 h后联合检测 H-FABP、cTnI、BNP有利于提高AMI检测的敏感性和特异性,更加有利于临床早期诊断AMI。
目的:探討聯閤檢測心髒型脂肪痠結閤蛋白(H-FABP)、心肌肌鈣蛋白I(cTnI)、腦鈉肽(BNP)在急性心肌梗死(AMI)早期診斷中的意義。方法選擇以急診入院的患者116例,其中 AMI患者64例,非AMI患者52例。用膠體金方法檢測患者髮病0~3 h和3~12 h血清中 H-FABP ;用化學髮光分析方法定量檢測cTnI、BNP的濃度,用統計學方法分析 H-FABP、cTnI、BNP項目的敏感性和特異性。結果 AMI組髮病0~3 h測定的 H-FABP、cTnI、BNP的敏感性分彆是95.45%、59.09%、61.36%;髮病3~12 h後測定的 H-FABP、cTnI、BNP的敏感性和特異性分彆是97.73%、97.73%、86.36%和90.63%、84.38%、81.25%。結論髮病0~3 h檢測 H-FABP對AMI敏感性較高,具有早期診斷價值,在髮病3~12 h後聯閤檢測 H-FABP、cTnI、BNP有利于提高AMI檢測的敏感性和特異性,更加有利于臨床早期診斷AMI。
목적:탐토연합검측심장형지방산결합단백(H-FABP)、심기기개단백I(cTnI)、뇌납태(BNP)재급성심기경사(AMI)조기진단중적의의。방법선택이급진입원적환자116례,기중 AMI환자64례,비AMI환자52례。용효체금방법검측환자발병0~3 h화3~12 h혈청중 H-FABP ;용화학발광분석방법정량검측cTnI、BNP적농도,용통계학방법분석 H-FABP、cTnI、BNP항목적민감성화특이성。결과 AMI조발병0~3 h측정적 H-FABP、cTnI、BNP적민감성분별시95.45%、59.09%、61.36%;발병3~12 h후측정적 H-FABP、cTnI、BNP적민감성화특이성분별시97.73%、97.73%、86.36%화90.63%、84.38%、81.25%。결론발병0~3 h검측 H-FABP대AMI민감성교고,구유조기진단개치,재발병3~12 h후연합검측 H-FABP、cTnI、BNP유리우제고AMI검측적민감성화특이성,경가유리우림상조기진단AMI。
Objective To explore the value of combined detection of heart-type fatty acid binding protein (H-FABP) ,cardiac troponin I(cTnI) and brain natriuretic peptide(BNP) in the early diagnosis of acute myocardial in-farction(AMI) .Methods A total of 116 cases with emergency admission into this hospital were enrolled ,including 64 cases of AMI and 52 cases of non-AMI .Serum H-FABP were detected by using colloidal gold method ,and levels of cTnI and BNP were detected by using chemiluminescence method .The sensitivity and specificity of H-FABP ,cTnI and BNP were statistically analyzed .Results Within 0-3 h after onset of AMI ,the sensitivity of H-FABP ,cTnI and BNP were 95 .45% ,59 .09% and 61 .36% ,respectively .Within 3 -12 h after onset of AMI ,the sensitivity of H-FABP ,cTnI and BNP were 97 .73% ,97 .73% and 86 .36% ,and specificity were 90 .63% ,84 .38% and 81 .25% ,re-spectively .Conclusion Within 0-3 h after onset ,H-FABP could be with higher sensitivity in the diagnosis of AMI , which might have great significance for early diagnosis .Within 3-12 h after onset ,combined detection of H-FABP , cTnI and BNP could improve the sensitivity and specificity for the diagnosis of AMI ,which might be more helpful for early diagnosis .
