目的：评价128层螺旋CT数字减影血管成像对颅内动脉瘤的诊断价值。方法60例临床怀疑颅内动脉瘤的患者分别进行128层螺旋CT数字减影血管成像（ Digital subtraction angiography ，DSACTA）和数字减影血管造影（ DSA），并对照手术或介入栓塞治疗结果评估其诊断价值。结果 DSACTA共发现58例66个动脉瘤，其中52例为单个动脉瘤，4例为两个动脉瘤，2例多发动脉瘤。动脉瘤直径最小2 mm，最大30 mm。DSA检出60例68个动脉瘤，以DSA检出结果为金标准，128层螺旋CT数字减影血管成像，诊断符合率97.1％，2例2个小动脉瘤漏诊，漏诊率2.9％。结论 DSACTA诊断颅内动脉瘤有较高准确性，可部分取代DSA造影检查。
목적：평개128층라선CT수자감영혈관성상대로내동맥류적진단개치。방법60례림상부의로내동맥류적환자분별진행128층라선CT수자감영혈관성상（ Digital subtraction angiography ，DSACTA）화수자감영혈관조영（ DSA），병대조수술혹개입전새치료결과평고기진단개치。결과 DSACTA공발현58례66개동맥류，기중52례위단개동맥류，4례위량개동맥류，2례다발동맥류。동맥류직경최소2 mm，최대30 mm。DSA검출60례68개동맥류，이DSA검출결과위금표준，128층라선CT수자감영혈관성상，진단부합솔97.1％，2례2개소동맥류루진，루진솔2.9％。결론 DSACTA진단로내동맥류유교고준학성，가부분취대DSA조영검사。
Objective To evaluate the diagnostic value of 128-slice spiral CT digital subtraction cerebral angiogra-phy for intracranial aneurysm.Methods 128-slice spiral CT digital subtraction cerebral angiography ( DSACTA) and digital subtraction angiography ( DSA) were performed in 60 patients with suspected intracranial aneurysm , and the diag-nostic value was evaluated compared with the finding of operations or the outcome of interventional embolization thera -py.Results A total of 66 aneurysms were detected via DSACTA in 58 cases, and there were 52 patients with single an-eurysm, 4 patients with 2 aneurysms, 2 patients with multiple aneurysms.Whose minimum diameter was 2mm and maxi-mum diameter was 30mm.68 aneurysms were detected in 60 cases via DSA.according to the result of DSA , the diagnos-tic coincidence of DSACTA was 97.1%and omission diagnose rate was 2.9%.Conclusions DSACTA could substitute DSA partly for its high accuracy.
