目的:观察盲肠结扎穿孔( CLP)所致脓毒症大鼠肺组织渗漏、血浆血管生成素-2( Ang-2)的变化,并探讨两者的关系。方法18只SD大鼠随机分为正常组6只和脓毒症组12只。正常组不造模;脓毒症组用CLP制作脓毒症模型,并于CLP后12、24 h时各取6只处死。观察两组大鼠肺组织病理、肺湿干重比、支气管肺泡灌洗液( BALF)中总蛋白浓度及血浆Ang-2浓度。结果脓毒症组CLP后12 h时血浆Ang-2浓度高于正常组,差异有统计学意义;随着时间延长,脓毒症组肺组织损伤病理改变进行性加重,肺湿干重比、BALF中总蛋白浓度和血浆Ang-2浓度呈升高趋势;肺湿干重比、BALF中总蛋白浓度与血Ang-2浓度均呈正相关( r=0.627,r=0.755,均P<0.05)。结论 CLP所致脓毒症大鼠肺渗透性增加与血中的Ang-2有关,这提示针对阻断Ang-2及其通路下游靶点的相关干预措施有望减轻脓毒症患者肺组织渗漏。

목적:관찰맹장결찰천공( CLP)소치농독증대서폐조직삼루、혈장혈관생성소-2( Ang-2)적변화,병탐토량자적관계。방법18지SD대서수궤분위정상조6지화농독증조12지。정상조불조모;농독증조용CLP제작농독증모형,병우CLP후12、24 h시각취6지처사。관찰량조대서폐조직병리、폐습간중비、지기관폐포관세액( BALF)중총단백농도급혈장Ang-2농도。결과농독증조CLP후12 h시혈장Ang-2농도고우정상조,차이유통계학의의;수착시간연장,농독증조폐조직손상병리개변진행성가중,폐습간중비、BALF중총단백농도화혈장Ang-2농도정승고추세;폐습간중비、BALF중총단백농도여혈Ang-2농도균정정상관( r=0.627,r=0.755,균P<0.05)。결론 CLP소치농독증대서폐삼투성증가여혈중적Ang-2유관,저제시침대조단Ang-2급기통로하유파점적상관간예조시유망감경농독증환자폐조직삼루。
Objective To observe the changes of lung permeability and serum angiopoietin -2 ( Ang-2) in ce-cal ligation and puncture ( CLP) induced sepsis in rats , and thus to investigate the correlation between serum Ang -2 and lung permeability.Methods Eighteen SD rats were randomly divided into control group (n=6) and sepsis group (n=12).The rats in sepsis group were modeled by CLP .Each 6 rats in sepsis group were sacrificed 12 h and 24 h after CLP, for assessment of the histological changes of lung , lung wet to dry weight ratio (W/D), total protein in bronchoalveolar lavage ( BALF) and serum Ang-2 concentrations .Results Serum Ang-2 concentration of rats in sepsis group was high-er than that in control group 12 h after CLP.In the sepsis group, with the time increased after CLP, the histological chan-ges were strengthened , and the lung W/D, total protein and serum Ang -2 concentration were also increased .Further-more, there was significantly positive correlation between serum Ang -2 and either W/D or total protein in BALF ( r=0.627, r=0.755, both P<0.05).Conclusion The increased lung permeability in sepsis rats induced by CLP is re-lates to blood Ang-2, which indicates that intervention that blocking the Ang -2 or related targets downstream its path-ways were helped to attenuate the lung leak in septic patients .
