目的 观察胃旁路术(GBP)对Ⅱ型糖尿病大鼠血糖及血清胰高血糖素样肽-1(GLP-1)的影响.方法 对80只雄性SD大鼠腹腔注射链脲佐菌素建立2型糖尿病模型.将建模成功的大鼠分成手术组18只(O组);假手术组15只(S组);饮食控制组15只(F组);对照组15只(C组).检测术前及术后1、2、4、8周的体质量、空腹血糖、空腹血浆胰岛素水平、空腹血清GLP-1浓度.结果 (1)O组术后空腹血糖第8周降至(8.2±2.4)mmol/L,与术前比较差异有统计学意义(P<0.05).(2)O组术后空腹血浆胰岛素8周后从术前(29.1±5.1)mU/L升至(72.5 ±7.4) mU/L(P <0.05).(3)O组术后GLP-1明显升高,到第8周上升至(23.26±2.19) pmol/L(P<0.05).结论 胃旁路手术可以有效降低2型糖尿病大鼠的血糖并引起GLP-1的高分泌,其血糖下降与限制饮食及体质量下降无明显相关.

Objective To investigate the effect of gastric bypass (GBP) on rats with type 2 diabetes induced by streptozotocin (STZ) injection and the action mechanisms involved.Methods Twenty rats were divided into 4 groups:operation group (group O,n =18),sham operation group (group S,n =15),food control group (group F,n =15) and control group (group C,n =15).Weight,fasting blood glucose (FBG),fasting plasma insulin (FPI) and glucagon-like peptide-1 (GLP-1) were measured 1,2,4 and 8 weeks after operation.Results (1) FBG in O group 8 weeks after operation was reduced to (8.2 ±2.4) mmol/L (P < 0.05) ; (2) FPI in O group was increased after operation and reached (72.5 ±7.4) mU/L at week 8 was compared with that before operation [(29.1 ± 5.1) mU/L] ; (3) GLP-1 in O Group at 8th week after operation was increased to (23.26 ± 2.19) pmoL/L(P <0.05).Conclusion GBP was an effective therapy to control blood glucose in type 2 diabetic rats and lead to high secretion of GLP-1.There is no significant relation between decreased blood glucose in type 2 diabetic rats with restricted diet and weight loss after GBP.
