目的:研究联合血清人绒毛膜促性腺激素(β-HCG)检测及胎心监护两种方法对胎儿窘迫的预测价值。方法将2012年1月至2013年3月本院足月妊娠孕妇710例进行胎心监护,根据监护结果分为3组:反应组、可疑组、无反应组。再根据血清β-HCG检测结果将每组分为两个亚组:血清β-HCG高者为1亚组,血清β-HCG低者为2亚组。结果无反应1组的胎儿窘迫发生率明显高于其他组(P=0.000<0.01)。联合使用胎心监护和血清β-HCG检测的敏感性、特异性、阳性预测率分别为92.5%、91.7%、93.6%,明显高于单独使用胎心监护和血清β-HCG检测(P<0.05)。结论联合使用血清β-HCG及胎心监护可以提高胎儿窘迫的诊断率,具有极高的预测价值。

Objective To study the serum β-HCG and fetal heart of two methods for detection of the predictive value of fetal distress. Method From January 2012 to March 2013 in our hospital 710 cases of full-term pregnancy, fetal heart examination to all pregnant women, according to the detection results were divided into three groups, response group, suspicious group, non response group. According to the test results of serumβ-HCG level of each group was divided into two sub groups, the serumβ-HCG as high as 1 sub groups, the serumβ-HCG as low as 2 sub group. Result No reactions of the 1 groups of the highest incidence of fetal distress, signiifcantly higher than other groups (P=0.000<0.01). The combined use of fetal heart and serumβ-HCG was detected in the sensitivity, speciifcity, positive predictive rate of three aspects respectively were 92.5%, 91.7%, 93.6% higher than single use of fetal heart and serum β-HCG detection (P < 0.05). Conclusion The combined use of serumβ-HCG and fetal heart examination can improve the diagnosis of fetal distress, predictive value.
