目的：探讨腹腔镜下保留睾丸动脉及淋巴管的精索内静脉高位结扎术治疗精索静脉曲张的效果。方法回顾分析65例行腹腔镜下保留睾丸动脉及淋巴管的精索内静脉高位结扎术(Artery-Lymphatics Sparing Laparoscopic Varicocelectomy，ALSLV)的原发性精索静脉曲张患者临床资料。结果65例手术均成功，手术时间为（38.5±6.5）min。术后平均随访1.5~2.5年，精液参数提高率72.3%（47/65）；自然妊娠率52.3%（34/65）；睾丸疼痛发生率为0（0/65）；睾丸萎缩发生率为0（0/65）；鞘膜积液发生率为0(0/65)；复发率为6.1%(4/65)。结论腹腔镜下保留睾丸动脉及淋巴管的精索内静脉高位结扎术治疗精索静脉曲张术手术可行，术后并发症发生率低，精液质量改善明显，值得临床推广。
목적：탐토복강경하보류고환동맥급림파관적정색내정맥고위결찰술치료정색정맥곡장적효과。방법회고분석65례행복강경하보류고환동맥급림파관적정색내정맥고위결찰술(Artery-Lymphatics Sparing Laparoscopic Varicocelectomy，ALSLV)적원발성정색정맥곡장환자림상자료。결과65례수술균성공，수술시간위（38.5±6.5）min。술후평균수방1.5~2.5년，정액삼수제고솔72.3%（47/65）；자연임신솔52.3%（34/65）；고환동통발생솔위0（0/65）；고환위축발생솔위0（0/65）；초막적액발생솔위0(0/65)；복발솔위6.1%(4/65)。결론복강경하보류고환동맥급림파관적정색내정맥고위결찰술치료정색정맥곡장술수술가행，술후병발증발생솔저，정액질량개선명현，치득림상추엄。
Objective To investigate the clicinal ef icacy of ALSLV in the treatment of varicocele. Methods The clinical documents of 65 cases of patients with primary varicocele treated with ALSLV were retrospective analized. Results Al the operation of 65 cases were successful and were fol owed-up 1.5-2.5 years after operation. The operatig time were (38.5±6.5)min,the improving rates of semen quality in 6 months were 72.3%(47/65);the spontaneous pregnant rates of his spouses in 2 years were 52.3%(34/65),the incidences of testicular hydrocele were 0 (0/65);the incidences of testicular orchiatrophy were 0 (0/65),the incidences of testicular pain were 0(0/65).The recurrence rates were6.1%(4/65). Conclusion ALSLV has less complications and high improving rates of semen quality in the treatment of primary varicocele,It is a promising method for the treatment of primary varicocele.
