目的：探讨密闭式吸痰（CSS）与开放式吸痰（OSS）对新生儿呼吸机相关性肺炎（ventilator-associated pneumonia ，VAP）发生率的影响，为临床选择最佳吸痰方式提供依据。方法选择2011年3月～2012年12月在我院新生儿重症监护病房（NICU ）经口行气管插管机械通气治疗患儿100例，随机分为CSS组（实验组）和OSS组（对照组）各50例，比较分析两组患者的呼吸机相关性肺炎发生率、病死率、住院时间、住院费用及菌群情况等。结果实验组呼吸机相关性肺炎发生率、病死率均低于对照组，住院时间短于对照组，两组患儿VAP发生率差异有显著意义（P＜0．05），两组患儿呼吸道内培养出的菌株以革兰阴性菌为主，偶有真菌；OSS组的菌株种类杂、数量多。结论密闭式吸痰方法在降低新生儿呼吸机相关性肺炎中的效果优于开放式吸痰法，值得临床推广使用。

목적：탐토밀폐식흡담（CSS）여개방식흡담（OSS）대신생인호흡궤상관성폐염（ventilator-associated pneumonia ，VAP）발생솔적영향，위림상선택최가흡담방식제공의거。방법선택2011년3월～2012년12월재아원신생인중증감호병방（NICU ）경구행기관삽관궤계통기치료환인100례，수궤분위CSS조（실험조）화OSS조（대조조）각50례，비교분석량조환자적호흡궤상관성폐염발생솔、병사솔、주원시간、주원비용급균군정황등。결과실험조호흡궤상관성폐염발생솔、병사솔균저우대조조，주원시간단우대조조，량조환인VAP발생솔차이유현저의의（P＜0．05），량조환인호흡도내배양출적균주이혁란음성균위주，우유진균；OSS조적균주충류잡、수량다。결론밀폐식흡담방법재강저신생인호흡궤상관성폐염중적효과우우개방식흡담법，치득림상추엄사용。
Objective To study of the effect on occurrence of neonatal ventilator-associated pneumonia (VAP) by closed endotracheal suction systems(CSS)and open endotracheal suction systems (OSS) ,and to provide the basis for selecting best suctioning approach for clinical using .Method 100 neonates received intubation and mechanical ventila-tion in neonatal intensive care unit (NICU) of Yu Lin Maternal and Child Health-Care Hospital from March 2011 to December 2012 was selected and randomly divided into CSS group (treatment group ) and OSS group (control group) ,each with 50 neonates .The incidences of VAP ,mortality ,the length and cost of hospital stay ,bacterial contamination was measured and compared between two groups .Result Compared to control group ,neonates in treatment group had lower incidences of VAP and mortality ,and shorter hospital stay .There was significant differ-ences between the two groups (P<0 .05) .Gram-negative bacteria were in the majority of strains cultured from neo-nate respiratory tract in both groups .Further ,more species and larger number of strains were observed in OSS group .Conclusion CSS was superior to OSS for reducing the incidence VAP for neonates received intubation and me-chanical ventilation .It's valuable for extending in clinical practice .
