目的:研究磁共振弥散加权成像(diffusion weighted imaging,DWI)和ADC图在脑梗死分期诊断中的临床应用价值。方法收集经临床及影像学证实的62例患者的DWI及ADC图资料,其中,超急性期12例,急性期38例,亚急性期12例。结果62例患者中,发生在脑干的2例超急性期腔隙性脑梗死3 h~4 h内未能明确诊断,约占3.2%,其余不同时期梗死病灶在DWI高信号,ADC图低信号,随时间延长,DWI信号逐渐降低,ADC信号升高,2周时接近等信号。结论 MRI检查是诊断缺血性脑梗死的有效方法,根据DWI和ADC图像信号特点,可以鉴别新、旧梗死并确定责任病灶,为临床治疗提供有力的证据。

목적:연구자공진미산가권성상(diffusion weighted imaging,DWI)화ADC도재뇌경사분기진단중적림상응용개치。방법수집경림상급영상학증실적62례환자적DWI급ADC도자료,기중,초급성기12례,급성기38례,아급성기12례。결과62례환자중,발생재뇌간적2례초급성기강극성뇌경사3 h~4 h내미능명학진단,약점3.2%,기여불동시기경사병조재DWI고신호,ADC도저신호,수시간연장,DWI신호축점강저,ADC신호승고,2주시접근등신호。결론 MRI검사시진단결혈성뇌경사적유효방법,근거DWI화ADC도상신호특점,가이감별신、구경사병학정책임병조,위림상치료제공유력적증거。
Objective To research on the clinical applying value of using Diffusion Weighted Imaging(DWI)and ADC chart for the cerebral infarction classification diagnosis. Methods All cases collected from 62 patient's DWI and ADC charts which had been testified in clinic and by iconography. These patients are consisted of 12 hyper-acute stage cases,38 acute stage cases,12 sub-acute stage cases. Results Two hyper-acute stage cases of lacunars infarction that was occurred in brain stem can not be diagnosed definitely in 3~4 hours and this took up by 2.8 percentages. But the cerebral infarction focuses of other stage showed high signal in DWI and low signal in ADC chart. But with the time passing,DWI signal would decline gradually and ADC signal had increased in contrary,and was close to equal signal when 2W.Conclusion The MRI examination is an effective way to diagnose ischemic brain infarction. And according to the features of DWI and ADC signal,we can identify the new or old infarcts and certify the main focus. Therefore,this kind of unique signal can provide convincing evidence for treatment in clinic.
