目的:探讨微量元素硒与特发性脊柱侧弯的相关性,为特发性脊柱侧弯的病因学研究提供依据。方法:选取山西医科大学第二医院90例青少年特发性脊柱侧弯患者为病例组,Cobb角20°~85°,根据Cobb角的大小将患者分为三组:轻度组35例,中度组30例,重度组25例;以高发区同性别、同年龄段体检正常的青少年为对照组,共30例。采用荧光法测定血硒含量,病例对照研究分析硒含量与特发性脊柱侧弯的相关性。结果:病例组硒含量为(34.56±15.54)μg/L,明显低于对照组的(63.53±19.08)μg/L(P<0.01);轻、中、重三组硒含量与对照组比较均有统计学意义(P<0.01),且重度组显著低于轻、中度组(P<0.01);但轻、中度组之间比较差异无统计学意义(P>0.05);特发性脊柱侧弯患者血硒含量值与Cobb角相关系数为0.671(P<0.01)。结论:特发性脊柱侧弯患者血硒含量降低,且随着侧弯程度加重,硒缺乏表现更突出,微量元素硒的变化不是偶然的,两者可能存在相关性。

목적:탐토미량원소서여특발성척주측만적상관성,위특발성척주측만적병인학연구제공의거。방법:선취산서의과대학제이의원90례청소년특발성척주측만환자위병례조,Cobb각20°~85°,근거Cobb각적대소장환자분위삼조:경도조35례,중도조30례,중도조25례;이고발구동성별、동년령단체검정상적청소년위대조조,공30례。채용형광법측정혈서함량,병례대조연구분석서함량여특발성척주측만적상관성。결과:병례조서함량위(34.56±15.54)μg/L,명현저우대조조적(63.53±19.08)μg/L(P<0.01);경、중、중삼조서함량여대조조비교균유통계학의의(P<0.01),차중도조현저저우경、중도조(P<0.01);단경、중도조지간비교차이무통계학의의(P>0.05);특발성척주측만환자혈서함량치여Cobb각상관계수위0.671(P<0.01)。결론:특발성척주측만환자혈서함량강저,차수착측만정도가중,서결핍표현경돌출,미량원소서적변화불시우연적,량자가능존재상관성。
Objective:To explore the correlation between trace element Se and idiopathic scoliosis and provide the basis for idiopathic scoliosis' etiology. Method:90 patients with idiopathic scoliosis were examined and divided into three subgroups according to the Cobb angle:35 mild cases,30 moderate cases and 25 severe cases. 30 healthy adolescents of the same gender,age were recruited from high incidence areas. Fluorescence method was adopted to determine the selenium concentration in the whole blood,researched the correlation between selenium and idiopathic scoliosis by a case-control study. Result:Selenium concentration in the case group was(34.56±15.54)μg/L,and was lower than(63.53±19.08) μg/L in the control group,the difference was significantly(P<0.01);and selenium concentration in the severe group was significantly lower than the mild and moderate groups(P<0.01),but there was no statistical difference between the two groups (P>0.05). The correlation coefficient between selenium concentration and Cobb Angle was 0.671. Conclusion:Selenium concentration in scoliotic patients decreaseds,and with the severity of idiopathic scoliosis progressing,selenium deficiency become increasingly prominent. The changes of selenium concentration were not accidental,there is probably a correlation between them.
