[目的]探讨结缔组织生长因子（CTGF）、肿瘤坏死因子-α（TNF-α）在糖尿病肾病（DN）患者不同阶段的表达水平及相关性。[方法]根据24 h尿白蛋白排泄率率（UAER）将94例DN 患者分为无蛋白尿（NRU组）、微量蛋白尿（MIU组）及大量蛋白尿（MAU组）三组，同时以89例健康体检者作为对照组（CON组），检测各组血清中CTGF、TNF-α水平并比较分析。[结果]NRU 组、MIU 组及MAU 组血清CTGF和TNF-α水平显著高于 CON 组，其差异有统计学意义（ P ＜0．01）。 NRU 组、MIU 组及 MAU 组患者中UAER与CTGF（ r ＝0．924，P ＜0．01）和TNF-α（ r ＝0．861，P ＜0．01）呈正相关；血清中CTGF与 TNF-α水平呈正相关（ r ＝0．432，P ＜0．05）。[结论]血清中CTGF和TNF-α水平在DN患者中均有不同程度的增高，与UAER呈正相关，有可能成为监测DN病情变化的有效指标。
[목적]탐토결체조직생장인자（CTGF）、종류배사인자-α（TNF-α）재당뇨병신병（DN）환자불동계단적표체수평급상관성。[방법]근거24 h뇨백단백배설솔솔（UAER）장94례DN 환자분위무단백뇨（NRU조）、미량단백뇨（MIU조）급대량단백뇨（MAU조）삼조，동시이89례건강체검자작위대조조（CON조），검측각조혈청중CTGF、TNF-α수평병비교분석。[결과]NRU 조、MIU 조급MAU 조혈청CTGF화TNF-α수평현저고우 CON 조，기차이유통계학의의（ P ＜0．01）。 NRU 조、MIU 조급 MAU 조환자중UAER여CTGF（ r ＝0．924，P ＜0．01）화TNF-α（ r ＝0．861，P ＜0．01）정정상관；혈청중CTGF여 TNF-α수평정정상관（ r ＝0．432，P ＜0．05）。[결론]혈청중CTGF화TNF-α수평재DN환자중균유불동정도적증고，여UAER정정상관，유가능성위감측DN병정변화적유효지표。
[Objective] To explore the expression of connective tissue growth factor (CTGF) and tumor necrosis factor-alpha(TNF-α) in patients with diabetes nephropathy (DN) .[Methods]According to 24h urine albumin excre-tion rate(UAER) ,94 DN patients were divided into non-proteinuria group(NRU group) ,microalbuminuria group (MIU group) and mass proteinuria group(MAU group) .Other 89 healthy subjects were taken as the control group . Serum CTGF and TNF-αof each group were measured and compared .[Results]Serum levels of CTGF and TNF-αin NRU ,MIU and MAU groups were markedly higher than those in the control group ,and there was significant differ-ence( P<0 .01) .UAER in NRU ,MIU and MAU groups was positively correlated with CTGF ( r=0 .924 ,P<0 . 01) and TNF-α( r =0 .861 ,P<0 .01) .Serum CTGF was positively correlated with TNF-α( r =0 .432 ,P <0 . 05) .[Conclusion]Serum levels of CTGF and TNF-αin DN patients increase at different extent ,and is positively cor-related with UAER ,and may become the effective index to monitor the condition change of DN .
