目的:探讨过敏性紫癜(HSP)患儿食物不耐受情况。方法采用ELISA,检测比较该地区58例HSP患儿以及23名正常儿童血清中14种食物特异性IgG (sIgG)水平,并分析不同性别、年龄患儿的不耐受阳性率及不耐受食物的分布差异。结果58例患儿食物 sIgG 阳性率为84.5%,高于对照组的13.0%,比较差异有统计学意义(P<0.01)。HSP患儿不耐受的食物前4位依次为:鸡蛋/蛋白蛋黄(70.7%)、牛奶(53.4%)、鳕鱼(19.0%)、螃蟹(10.3%);其中1种食物sIgG阳性18例(31.0%),2种或2种以上食物sIgG阳性31例(53.4%)。不同性别、不同年龄[低龄儿童(<7岁)及高龄儿童(≥7岁)]食物sIgG总阳性率比较差异无统计学意义(P>0.05),高龄儿童的螃蟹IgG阳性率较低龄儿童高(P<0.01)。结论HSP患儿食物不耐受情况较正常儿童明显。该组HSP患儿对鸡蛋/蛋白蛋黄、牛奶、鳕鱼、螃蟹不耐受情况较为明显,可作为地区参考,高龄儿童对螃蟹的不耐受情况较低龄儿童明显。

목적:탐토과민성자전(HSP)환인식물불내수정황。방법채용ELISA,검측비교해지구58례HSP환인이급23명정상인동혈청중14충식물특이성IgG (sIgG)수평,병분석불동성별、년령환인적불내수양성솔급불내수식물적분포차이。결과58례환인식물 sIgG 양성솔위84.5%,고우대조조적13.0%,비교차이유통계학의의(P<0.01)。HSP환인불내수적식물전4위의차위:계단/단백단황(70.7%)、우내(53.4%)、설어(19.0%)、방해(10.3%);기중1충식물sIgG양성18례(31.0%),2충혹2충이상식물sIgG양성31례(53.4%)。불동성별、불동년령[저령인동(<7세)급고령인동(≥7세)]식물sIgG총양성솔비교차이무통계학의의(P>0.05),고령인동적방해IgG양성솔교저령인동고(P<0.01)。결론HSP환인식물불내수정황교정상인동명현。해조HSP환인대계단/단백단황、우내、설어、방해불내수정황교위명현,가작위지구삼고,고령인동대방해적불내수정황교저령인동명현。
Objective To study food hypersensitivity of children with Hennch-Sehonlein purpura (HSP).Methods The enzyme-linked immunosorbent assay (ELISA)was used to detect 1 4 kinds of food allergen-specific IgG (sIgG)levels in 58 children with HSP and 23 healthy children.Results Eighty-four point five percent(49/58)children with HSP showed food allergen-specific IgG positive(84.5%)higher than healthy children(1 3.0%)(P<0.05).Egg(70.7%),milk(53.4%),codfish(1 9.0%)and crab(1 0.3%) were the main allergens in HSP.Eighteen cases (31 .0%)with HSP were found to have one type of food aller-gen-specific lgG increment,31 cases (53.4%)with two or more types of sIgG increment.The total positive rate of food sIgG showed no difference between sex and age groups (P>0.05).There was higher positive rate of crab IgG in the older children(≥7 years old)(P<0.05).Conclusions Food hypersensitivity of children with HSP is more obvious than healthy children.The mainly allergens of these children were egg,milk,cod-fish and crab,which could be regarded as region refernce.The older children is more sensitive to crab.
