目的:探讨各种原因导致配血不相合的解决办法。方法对我院的61例交叉配血不相合的26例标本进行血型鉴定、意外抗体鉴定、献血者标本检测等免疫血清学分析,查找不相合原因,针对配血不相合原因进行不同方法的交叉配血,并对各种交叉配血不相合原因及同种抗体鉴定结果进行统计分析。结果多种原因可导致交叉配血结果不相合,其中人为因素(操作失误)11例,血型鉴定困难11例,献血者原因17例,患者同种抗体17例,自身抗体5例。抗-E在同种抗体中的比例为12/17。61例交叉配血不相合患者标本经过正确处理后均找到了相合的血液输注,均未发生输血反应。结论交叉配血不相合时应在保证消除人为因素(操作失误)的前提下综合分析原因,并选用相适应方法进行交叉配血,才能保障临床输血安全有效。

목적:탐토각충원인도치배혈불상합적해결판법。방법대아원적61례교차배혈불상합적26례표본진행혈형감정、의외항체감정、헌혈자표본검측등면역혈청학분석,사조불상합원인,침대배혈불상합원인진행불동방법적교차배혈,병대각충교차배혈불상합원인급동충항체감정결과진행통계분석。결과다충원인가도치교차배혈결과불상합,기중인위인소(조작실오)11례,혈형감정곤난11례,헌혈자원인17례,환자동충항체17례,자신항체5례。항-E재동충항체중적비례위12/17。61례교차배혈불상합환자표본경과정학처리후균조도료상합적혈액수주,균미발생수혈반응。결론교차배혈불상합시응재보증소제인위인소(조작실오)적전제하종합분석원인,병선용상괄응방법진행교차배혈,재능보장림상수혈안전유효。
Objective To explore measures for incompatible cross-matching by analyzing 61 cases of incompatible cross-matching. Methods Samples with incompatible cross-matching in recent 2 years in our hospital were tested with blood grouping,unexpected antibody identification and serological tests of store blood. Tests of cross-matching based causes of inconformity of cross-matching were performed. Whether there was significant difference between ratio of anti-E in our test and reported 22/37 was analyzed. Results Several causes were found can induce inconformity of cross-matching. The amount of causes of incompatible cross-matching found among 61 cases were:artificial causation 11,unconformity of forward and reverse ABO blood grouping 11,disqualification of store blood 17,allo-antibody 17,auto-antibody 5. There was no significant difference between ratio of anti-E in our test and reported 12/17. No blood transfusion reactions were observed among 61 cases after compatible stored blood transfusion. Conclusion Analysis and management of causes for inconformity of cross-matching can ensure compatibility of stored blood and transfusion safety.
