Mycobacterium tuberculosis%Protein chip%Antibody
目的：探讨结核蛋白芯片检测在结核病诊断和疗效观察中的价值。方法：使用蛋白芯片系统检测血清结核抗体的病例共3676例，其中结核病例407例，非结核病例3269例。采用蛋白芯片技术检测结核杆菌中5种成分，即ESAT-6、CFP10、LAM、38KD和16KD，分析其结果。结果：407例结核病患者中共275例结核抗体阳性，3269例非结核病例547例阳性，结核蛋白芯片检测的灵敏度67.56%，特异度83.27%。对222例患者的治疗后抗体动态观察，开始治疗后每隔3个月抗体阳性人数未逐渐下降。结论：结核蛋白芯片是诊断结核病的较有效方法，具有特异性好、快速方便等优点，对结核病快速辅助诊断有积极作用。尤其对肺结核快速辅助诊断有积极作用，但在淋巴结核、结核性胸膜炎中阳性率偏低，敏感性稍差。对结核病的疗效监测无实用价值。

목적：탐토결핵단백심편검측재결핵병진단화료효관찰중적개치。방법：사용단백심편계통검측혈청결핵항체적병례공3676례，기중결핵병례407례，비결핵병례3269례。채용단백심편기술검측결핵간균중5충성분，즉ESAT-6、CFP10、LAM、38KD화16KD，분석기결과。결과：407례결핵병환자중공275례결핵항체양성，3269례비결핵병례547례양성，결핵단백심편검측적령민도67.56%，특이도83.27%。대222례환자적치료후항체동태관찰，개시치료후매격3개월항체양성인수미축점하강。결론：결핵단백심편시진단결핵병적교유효방법，구유특이성호、쾌속방편등우점，대결핵병쾌속보조진단유적겁작용。우기대폐결핵쾌속보조진단유적겁작용，단재림파결핵、결핵성흉막염중양성솔편저，민감성초차。대결핵병적료효감측무실용개치。
Objective:To investigate the value of tuberculosis protein chip detection in diagnosis and treatment observation of tuberculosis.Methods:3676 cases were given detection of serum anti Mycobacterium tuberculosis antibody by the protein chip system 407 cases of tuberculosis and 3269 cases of non tuberculosis.We Used the protein chip system to detecte of 5 components in mycobacterium tuberculosis,respectively was ESAT-6,CFP10,LAM,38KD and 16KD,and analyzed the results.Results:In 407 patients with tuberculosis,275 cases of tuberculosis antibody positive,in 3269 cases of non tuberculosis,547 cases of tuberculosis antibody positive,the sensitivity of tuberculosis protein chip was 67.56% ,and the specificity was 83.27% .In the dynamic observation on changes of anti-tuberculosis antibody in 222 patients,it was found that the number of the patients with positive reaction did not gradually decrease every 3 months after treatment.Conclusion:Tuberculosis protein chip is a useful and effective method for diagnosis of tuberculosis,also have the advantages of good specificity,rapid and convenient,and play a positive role in rapid diagnosis of tuberculosis,especially for the pulmonary tuberculosis.But it is less sensitive for lymph tuberculosis and pleurisy,and can not monitor the treatment effects of tuberculosis.
