目的:探讨3.0T磁共振在乳腺疾病中的应用。方法使用西门子3.0T磁共振仪和8通道乳腺线圈对32例乳腺疾病患者进行平扫加增强检查。结果所有患者诊断均结合平扫T1、T2、DWI及增强图像进行分析其病变的信号变化,并测量DWI-ADC值,24例良性患者在(1.82±0.30)×10-3mm2/s,8例恶性患者在(1.010±0.16)×10-3mm2/s;绘制DCE-TIC曲线,32例患者Ⅰ型20例,Ⅱ型5例,Ⅲ型7例;计算DWI-ADC值与病理诊断符合率(χ2=6.710,P<0.01),有统计学意义;比较DCE-TIC结果与病理诊断符合率(χ2=4.270,P<0.05),有统计学意义。结论应用3.0T磁共振检查乳腺病变,对确诊良、恶性具有较高的敏感性,但须在平扫基础上结合DWI和DCE来进行分析,才能增加诊断准确率。

Objective To discuss the application of 3.0 T MRI in breast diseases. Methods 32 patients with mammary gland disease were scaned and strengthen inspection with the Siemens 3.0T MRI and 8 channel breast coil. Results Combining the pathological signal of T1,T2,DWI and enhance image to diagnosis in all patients,and to measure DWI-ADC values;The ADC val-ues of 24 cases was (1.82+0.30) ×10-3mm2/s,and the ADC values of 8 patients was (1.010+0.16) ×10-3mm2/s;Drawing DCE-TIC curve,there are 20 cases withⅠtype in 32 patients,andⅡtype 5 cases,Ⅲtype 7 cases;Calculated the coincidence rate of ADC values and pathological diagnosis,(χ2=6.710,P<0.01), there was a statistical significance;Compared the coincidence rate of patho-logical diagnosis to the type of DCE-TIC(χ2=4.270,P<0.05),there was a statistical significance. Conclusion There was a high sen-sitivity in diagnosis the benign and malignant of breast disease with 3.0 T MRI,but it must be based on the DWI and DCE scan to increase the diagnostic accuracy.
