目的:探讨磁共振3D-B-FFE和3D-TOF序列联合应用对血管压迫性三叉神经痛的诊断价值。方法:对58例血管压迫性三叉神经痛的患者均使用磁共振3D-B-FFE和3D-TOF序列进行三叉神经脑池段脑干薄层扫描,并进行三维重建,从多角度共同观察患侧三叉神经脑池段及近脑干段与周围血管的三维空间关系,并与手术结果进行双盲对照。结果:58例患者中,56例发现责任血管,单独用3D-TOF 序列发现责任血管的阳性率83.33%;单独用3D-B-FFE序列发现责任血管的阳性率93.10%;联合应用3D-B-FFE和3D-TOF发现责任血管的阳性率96.55%,并与手术结果完全符合。结论:联合应用3D-B-FFE和3D-TOF序列能够清楚显示三叉神经脑池段与责任血管的空间关系,对于明确三叉神经痛的病因诊断及手术方式的选择具有重要的临床意义。

Objective:To investigate the value of MRI 3D-B-FFE combined with 3D-TOF sequence in diagnosis of trigeminal neuralgia caused by vscular compression.Methods:58 patients with trigeminal neuralgia caused by vscular compression were taken TLC scanning and 3D reconstruction on brainstem of trigeminal nerve cisternal segment using of magnetic resonance 3D-B-FFE and 3D-TOF sequences,also can observe the three-dimensional space between the segment of trigeminal nerve cisternal and proximal stem and the surrounding vessels in affected side from affected side,and contrasted with the operation using double blind method.Results:In 58 patients,56 cases were found offending vessel,the positive rate of finding offending vessel was 83.33% when using of 3D-TOF sequence alone;the positive rate of finding offending vessel was 93.10% when using of 3D-B-FFE sequence alone;the positive rate of finding offending vessel was 96.55% when combined 3D-B-FFE and 3D-TOF sequence,and in full compliance with the operation result.Conclusion:The application of 3D-B-FFE sequence combined with 3D-TOF sequence can clearly show the spatial relationship between the trigeminal nerve cisternal segment and the offending vessel.It has an important clinical significance in etiological diagnosis of trigeminal neuralgia and the choosing the operating way.
