目的：探讨关节镜下应用聚对二氧杂环已酮（PDS）线高张力固定前交叉韧带胫骨髁间嵴撕脱性骨折的临床疗效。方法选择2008年10月-2013年1月在喀什地区第一人民医院骨科住院的胫骨髁间嵴撕脱性骨折患者42例，在关节镜下应用 PDS 线将撕脱的前交叉韧带止点“十”字交叉固定于髁间嵴附着点处，同时于胫骨中上段应用空心钉为 PDS 线提供张力，加强固定撕脱的骨折块。采用 Lysholms-Tegner 膝关节功能评分分析临床疗效。结果42例随访时间3～12个月，平均6．5个月，Lysholms-Tegner 评分为（96．78±1．33）分。Lachman 试验均阴性，骨折愈合良好，无感染病例。结论关节镜下应用 PDS 线结合胫骨中上段空心钉高张力固定前交叉韧带胫骨髁间嵴撕脱性骨折是一种创伤小、操作简单的手术，可牢固固定骨折块，愈合率高，术后膝关节功能恢复快，值得推广应用。
목적：탐토관절경하응용취대이양잡배이동（PDS）선고장력고정전교차인대경골과간척시탈성골절적림상료효。방법선택2008년10월-2013년1월재객십지구제일인민의원골과주원적경골과간척시탈성골절환자42례，재관절경하응용 PDS 선장시탈적전교차인대지점“십”자교차고정우과간척부착점처，동시우경골중상단응용공심정위 PDS 선제공장력，가강고정시탈적골절괴。채용 Lysholms-Tegner 슬관절공능평분분석림상료효。결과42례수방시간3～12개월，평균6．5개월，Lysholms-Tegner 평분위（96．78±1．33）분。Lachman 시험균음성，골절유합량호，무감염병례。결론관절경하응용 PDS 선결합경골중상단공심정고장력고정전교차인대경골과간척시탈성골절시일충창상소、조작간단적수술，가뢰고고정골절괴，유합솔고，술후슬관절공능회복쾌，치득추엄응용。
Objective To explore the clinical effect of high tension PDS suture fixation for anterior cruciate ligament avulsion fracture from tibial eminence by arthroscopy monitoring.Methods From October 2008 to January 2013.We used “cross”PDS suture to fixed anterior cruciate ligament avulsion from tibial emi-nence under the monitor of arthroscopy in our hospital,as well as application hollow nail to provide high tension for the PDS sutue,strengthened the fixed of the tear fracture.According to Meyors-Mcerer classi-fication,the patients included 13 cases of type Ⅱ,29 cases of type Ⅲ.The knee joint score were evaluated according to the lysholms standard before and after operation.Results Forty five cases were followed up for 3 to 12 months,with the average of 6.5 months,the postoperative knee function score was (96.78± 1.33)months after operation,the Lachman test were all negative,and the fractures were all healed,with no cases of infection.Conclusion Combined with the screw fixation in upper to middle tibia provided high tension of PDS suture to fixed avulsion fractures of anterior cruciate ligament from tibial eminence is a minimally invasive,simple arthroscopic operation.Which can firmly fixed fracture,with high healing rate, and the knee joint function recovered quickly after operation.Which prove to be an operation worthy of popularization and application.
