目的:观察有创主动脉及桡动脉血压对老年冠心病Gensini评分的影响。方法选取同期行冠状动脉造影的老年高血压患者162例,根据冠状动脉狭窄程度将患者分为正常组(狭窄<50%)、冠心病组(狭窄≥50%)。冠状动脉造影时同时测定中心动脉压及外周动脉压,根据冠状动脉造影结果进行Gensini评分,并观察各种血压参数和冠状动脉病变严重程度之间的关系。结果冠心病组患者中心动脉脉压较正常组要大,差异有统计学意义[(74.4±27.4)mmHg vs.(69.5±26.2) mmHg,P<0.05],中心动脉脉压与冠状动脉Gensini评分有正相关性(r=0.316,P=0.007),中心动脉脉压和年龄有正相关性(r=0.462,P=0.005),冠状动脉Gensini评分和年龄有正相关性(r=0.298,P=0.013)。结论随着年龄增加,中心动脉脉压增大,增大的中心动脉脉压同冠状动脉粥样硬化病变程度有明显正相关性,可预测冠心病病变程度。

목적:관찰유창주동맥급뇨동맥혈압대노년관심병Gensini평분적영향。방법선취동기행관상동맥조영적노년고혈압환자162례,근거관상동맥협착정도장환자분위정상조(협착<50%)、관심병조(협착≥50%)。관상동맥조영시동시측정중심동맥압급외주동맥압,근거관상동맥조영결과진행Gensini평분,병관찰각충혈압삼수화관상동맥병변엄중정도지간적관계。결과관심병조환자중심동맥맥압교정상조요대,차이유통계학의의[(74.4±27.4)mmHg vs.(69.5±26.2) mmHg,P<0.05],중심동맥맥압여관상동맥Gensini평분유정상관성(r=0.316,P=0.007),중심동맥맥압화년령유정상관성(r=0.462,P=0.005),관상동맥Gensini평분화년령유정상관성(r=0.298,P=0.013)。결론수착년령증가,중심동맥맥압증대,증대적중심동맥맥압동관상동맥죽양경화병변정도유명현정상관성,가예측관심병병변정도。
ObjectiveTo explore the relationship between Gensini scores in old coronary artery disease patients and blood pressure derived from central aortic pressure or radial artery using invasive methods.Methods 162 patients were derived from who had coronary angiography with old hypertention patients at the same time. They were devided into CHD group and normal group depend on the severity of coronary artery stenosis. Central aortic pressure and radial artery pressure were used invasive methods. Gensini score was calculated for assessing the severity of CAD, and explore the relationship between blood pressure and Gensini scores which means the severity of CAD.ResultsPulse pressure of central aortic pressure was more bigger in CHD group than normal group [(74.4±27.4)mmHgvs.(69.5±26.2)mmHg,P<0.05], only pulse pressure of central aortic pressure and age were related to the Gensini scores(r=0.462, P=0.005), and Gensini scores were related to age (r=0.298,P=0.013).Conclusion Pulse pressure of central aortic pressure was increased with age increase, and this means the severity of CAD.
