目的：探讨乌拉地尔联合硝普钠治疗脑出血急性期高血压的降压效果及安全性。方法146例高血压性脑出血患者随机分为3组：硝普钠组49例与乌拉地尔组48例，分别持续静脉泵入硝普钠及乌拉地尔；硝普钠与乌拉地尔联合组49例，同时持续静脉泵入硝普钠及乌拉地尔。监测3组血压、心率，观察不良反应。结果3组均无不良反应，给药5 min时硝普钠组及联合组血压明显降低（P＜0.05），乌拉地尔组血压较治疗前无明显变化（P＞0.05），随后各组各时段与给药前相比血压降低，差异具有统计学意义（P＜0.05）。硝普钠组治疗后心率较治疗前增快（P＜0.05），乌拉地尔组及联合组治疗前后心率无明显变化，差异无统计学意义（均 P＞0.05）。结论联合应用硝普钠及乌拉地尔能够弥补单纯用硝普钠对心率的影响，并且较乌拉地尔降压快，能迅速控制脑出血急性期高血压。
목적：탐토오랍지이연합초보납치료뇌출혈급성기고혈압적강압효과급안전성。방법146례고혈압성뇌출혈환자수궤분위3조：초보납조49례여오랍지이조48례，분별지속정맥빙입초보납급오랍지이；초보납여오랍지이연합조49례，동시지속정맥빙입초보납급오랍지이。감측3조혈압、심솔，관찰불량반응。결과3조균무불량반응，급약5 min시초보납조급연합조혈압명현강저（P＜0.05），오랍지이조혈압교치료전무명현변화（P＞0.05），수후각조각시단여급약전상비혈압강저，차이구유통계학의의（P＜0.05）。초보납조치료후심솔교치료전증쾌（P＜0.05），오랍지이조급연합조치료전후심솔무명현변화，차이무통계학의의（균 P＞0.05）。결론연합응용초보납급오랍지이능구미보단순용초보납대심솔적영향，병차교오랍지이강압쾌，능신속공제뇌출혈급성기고혈압。
Objective To investigate the antihypertensive effect and safety of sodium nitroprusside combined with urapidil in treating hypertension in acute stage of cerebral hemorrhage. Methods 146 patients of hypertensive cerebral hemorrhage were randomly divided into three groups :sodium nitroprusside group (49 cases) and urapidil group (48 cases) ,as the control groups ,respectively ,continuous intravenous infusion of sodium nitroprusside or urapidil ;sodium nitroprusside combined with urapidil group (49 cases) ,continuous intravenous infusion of sodium nitroprusside and Urapidil ,as the study group. Blood pressure ,heart rate ,adverse reaction of the three groups were monitored and observed. Results There was no adverse reac-tion in all of these three groups. During the administration of the first 5 min ,the blood pressure of both sodium nitroprusside group and combination group was decreased significantly (P<0.05) ,the blood pressure of urapidil group had no significant changes (P>0.05).Then during each period of time ,the blood pressure of each groups was decreased compared with that be-fore the ad ministration ,and the difference was statistically significant (P<0.05).After treatment ,heart rate of sodium nitro-prusside group increased (P< 0.05) ,while heart rate had no significant change in urapidil group and combined treatment group ,with no significant difference (P> 0.05).Conclusion The combined use of sodium nitroprusside with urapidil could make up the simple effect of sodium nitroprusside on heart rate and could control the blood pressure faster compared with the single use of urapidill. They can quickly control the hypertension in the acute stage of cerebral hemorrhage.
