改良左肋緣下斜切口%手助腹腔鏡根治性腎切除術

Modified subcostal oblique incision%Hand assisted laparoscopic radical nephrectomy
目的:观察改良左肋缘下斜切口在手助腹腔镜根治性左肾切除术中的应用价值。方法2011年9月至2012年12月本科室对58例左肾肿瘤患者采用改良左肋缘下斜行切口作为手助切口,行手助腹腔镜根治性左肾切除术。其中,男36例,女22例,年龄42~74岁,平均56.3岁。肿瘤直径4.2~11cm,平均7.6cm。结果58例患者手术均成功完成,手术时间46~86分钟,平均58.5分钟。切口均获一期愈合,无一例出现腹部切口感染、脂肪液化等并发症。结论改良左肋缘下斜切口行手助腹腔镜根治性左肾切除术,有利于术者左手辅助操作暴露,减少术者手部不适,是一种较理想的左侧手助切口选择。

목적:관찰개량좌륵연하사절구재수조복강경근치성좌신절제술중적응용개치。방법2011년9월지2012년12월본과실대58례좌신종류환자채용개량좌륵연하사행절구작위수조절구,행수조복강경근치성좌신절제술。기중,남36례,녀22례,년령42~74세,평균56.3세。종류직경4.2~11cm,평균7.6cm。결과58례환자수술균성공완성,수술시간46~86분종,평균58.5분종。절구균획일기유합,무일례출현복부절구감염、지방액화등병발증。결론개량좌륵연하사절구행수조복강경근치성좌신절제술,유리우술자좌수보조조작폭로,감소술자수부불괄,시일충교이상적좌측수조절구선택。
Objective To evaluate the clinical application of the the Modified subcostal oblique incision in Hand assisted laparoscopic radical nephrectomy for the treatment of the left RCC .Methods From September 2011 to December 2012, 58 Left RCC patients were treated with Hand assisted laparoscopic radical nephrectomy by u-sing the Modified subcostal oblique incision as the hand assisted insicion .There were 36 males and 22 females, aged 42 to 74 years with an average age of 56.3 years, the diameter of the tumor size was 7.6(4.2~11) cm. Results All operations were performed successfully without conversion to open surgery .The mean operation time was 58.5(46~86) min.All incisions primarily healed .Conclusion As for the hand assisted incision in hand assisted laparoscopic radical nephrectomy of the left RCC , the Modified subcostal oblique incision can be helpful to expose the operation area and reduce the discomfort of the surgeon ’ s assisted left hand .
