目的：系统评价精氨酸强化的肠外营养（arginine-parenteral nutrition，Arg-PN）对结直肠癌患者免疫功能的影响。方法计算机检索中国期刊全文数据库（CNKI）、万方数据库、重庆维普数据库、会议和博士论文数据库、Medline和Embase数据库中关于Arg-PN对结直肠癌患者免疫功能影响的随机对照试验文献，评价纳入文献质量后应用RevMan 5.0.软件进行Meta分析。结果总计纳入文献8篇，均为针对Arg-PN的病例对照研究；Meta分析结果显示，经治疗一周后Arg-PN对细胞免疫水平的影响：试验组中 CD4＋、CD4＋/CD8＋水平高于对照组，CD8＋水平低于对照组，差异均具有统计学意义（P＜0.05）；对体液免疫水平的影响：试验组中IgM、IgG水平均高于对照组，差异亦具有统计学意义（P＜0.05）。结论 Arg-PN的强化肠外营养治疗对于结直肠癌患者免疫水平有正向调节作用，在未来的结直肠癌临床治疗实践中具有前瞻性的指导意义。
목적：계통평개정안산강화적장외영양（arginine-parenteral nutrition，Arg-PN）대결직장암환자면역공능적영향。방법계산궤검색중국기간전문수거고（CNKI）、만방수거고、중경유보수거고、회의화박사논문수거고、Medline화Embase수거고중관우Arg-PN대결직장암환자면역공능영향적수궤대조시험문헌，평개납입문헌질량후응용RevMan 5.0.연건진행Meta분석。결과총계납입문헌8편，균위침대Arg-PN적병례대조연구；Meta분석결과현시，경치료일주후Arg-PN대세포면역수평적영향：시험조중 CD4＋、CD4＋/CD8＋수평고우대조조，CD8＋수평저우대조조，차이균구유통계학의의（P＜0.05）；대체액면역수평적영향：시험조중IgM、IgG수평균고우대조조，차이역구유통계학의의（P＜0.05）。결론 Arg-PN적강화장외영양치료대우결직장암환자면역수평유정향조절작용，재미래적결직장암림상치료실천중구유전첨성적지도의의。
Objective This paper applied the Meta-analysis to review the impact of immune function on the colorectal cancer patients by the arginine parenteral nutrition (Arg-PN). Methods All associated RCT trails were collected in the database of CNKI, VIP, WANFANG, Medline, and Embase. The methodological quality of included studies was evaluated and the data was analyzed by RevMan 5.0 software. Results 8 trails were included in this study. The Meta-analysis results exhibited that the level on cellular immunity (CD4+, CD4+/CD8+) of the patients treated with Arg-PN was higher than on the control group, while the CD8+levels was lower, all the differences were statistically significant (P<0.05). On the impact of humoral immunity the level of IgM and IgG in experiment group was higher than in the control group, the differences were statistically significant (P<0.05). Conclusion Parenteral nutrition supplemented with arginine can improve the immune function of the patients with colorectal cancer.
