目的:探讨上皮性卵巢癌中癌相关成纤维细胞(CAFs)与临床病理特征间的相关性。方法应用免疫组化通用型二步法检测145例上皮性卵巢癌中平滑肌肌动蛋白(α-SMA)和基质金属蛋白酶-9(MMP-9)的表达,并分析两者的相关性及其临床意义。结果(1)正常卵巢组织除卵巢间质血管外,α-SMA不表达。在145例上皮性卵巢癌中,α-SMA阳性的CAFs细胞在癌组织中的分布有3种方式;且Ⅲ~Ⅳ期卵巢癌间质α-SMA阳性表达强度明显高于Ⅰ~Ⅱ期(P=0.016);有淋巴结转移的卵巢癌明显高于无淋巴结转移卵巢癌(P=0.049)。(2)正常卵巢组织中MMP-9不表达;而上皮性癌中癌细胞MMP-9阳性率为95.2%(138/145)。MMP-9在Ⅲ~Ⅳ期组明显高于Ⅰ~Ⅱ期组(P=0.028),有淋巴结转移组强阳性率高于无转移组(P=0.033)。(3)α-SMA在上皮性卵巢癌间质强阳性表达与癌细胞 MMP-9的表达呈正相关(P=0.027)。结论α-SMA 和 MMP-9两者的共同强阳性表达主要集中于Ⅲ~Ⅳ期癌患者和有淋巴结转移的患者中,CAFs的存在与数量增多将成为上皮性卵巢癌预后不良的一项独立指标。

목적:탐토상피성란소암중암상관성섬유세포(CAFs)여림상병리특정간적상관성。방법응용면역조화통용형이보법검측145례상피성란소암중평활기기동단백(α-SMA)화기질금속단백매-9(MMP-9)적표체,병분석량자적상관성급기림상의의。결과(1)정상란소조직제란소간질혈관외,α-SMA불표체。재145례상피성란소암중,α-SMA양성적CAFs세포재암조직중적분포유3충방식;차Ⅲ~Ⅳ기란소암간질α-SMA양성표체강도명현고우Ⅰ~Ⅱ기(P=0.016);유림파결전이적란소암명현고우무림파결전이란소암(P=0.049)。(2)정상란소조직중MMP-9불표체;이상피성암중암세포MMP-9양성솔위95.2%(138/145)。MMP-9재Ⅲ~Ⅳ기조명현고우Ⅰ~Ⅱ기조(P=0.028),유림파결전이조강양성솔고우무전이조(P=0.033)。(3)α-SMA재상피성란소암간질강양성표체여암세포 MMP-9적표체정정상관(P=0.027)。결론α-SMA 화 MMP-9량자적공동강양성표체주요집중우Ⅲ~Ⅳ기암환자화유림파결전이적환자중,CAFs적존재여수량증다장성위상피성란소암예후불량적일항독립지표。
Objective To investigate the relationship between the distribution of carcinoma-associated fibroblasts (CAFs) and their clinicopathological features as well as patient prognosis in epithelial ovarian cancer (EOC). Methods We used immunohistochemical technique to detect the expression of α-SMA and MMP-9 in 145 cases of EOC, and analyzed its relationship with clinicopathological features. Results The expression ofα-SMA in CAFs was upregulated as compared with normal tissue. Although the expression of α-SMA had no obvious relationship with age, tumor size and histological grade, it was associated with the clinical stage and lymph node metastases (P<0.05). The expression of MMP-9 in EOC cells was upregulated compared with normal tissue.The expression of MMP-9 was associated with the clinical stage and lymph node metastases (P<0.05), but had no obvious relationship with histological grade, tumor size and histological type (P>0.05). In addition, the expression ofα-SMA in CAFs was correlated with the expression of MMP-9 in cancer cells. Conclusion The CAFs in EOC seem to play an important role in the ovarian cancer progression and lymph node metastases. It may become potential prognostic predictors of EOC.
