目的:探讨脑钠肽与冠心病患者冠状动脉(冠脉)病变严重程度的相关性。方法入选行冠脉造影检查的冠心病患者173例,所有患者入院后均行冠脉造影检查及血浆脑钠肽(BNP)检查,分析不同冠脉病变支数、冠心病分型、冠脉病变程度(Gensini积分)与患者血清BNP水平之间的关系。结果随着冠脉病变支数增加,患者BNP水平呈显著增加趋势(P<0.05)。稳定型心绞痛、不稳定型心绞痛、心肌梗死三组患者BNP水平呈显著增加趋势,三组BNP水平之间差异有统计学意义(P<0.05)。BNP>100 pg/ml的患者Gensini积分显著高于BNP<100 pg/ml的患者Gensini积分,分别为(53.7±15.9)分vs.(29.3±20.7)分,差异具有统计学意义(P<0.05)。Gensini积分与BNP水平存在显著正相关(r=0.476,P<0.05)。结论 BNP水平与冠心病患者冠脉病变严重程度正相关,BNP水平有可能作为冠心病患者冠脉病变严重程度的判断指标。

목적:탐토뇌납태여관심병환자관상동맥(관맥)병변엄중정도적상관성。방법입선행관맥조영검사적관심병환자173례,소유환자입원후균행관맥조영검사급혈장뇌납태(BNP)검사,분석불동관맥병변지수、관심병분형、관맥병변정도(Gensini적분)여환자혈청BNP수평지간적관계。결과수착관맥병변지수증가,환자BNP수평정현저증가추세(P<0.05)。은정형심교통、불은정형심교통、심기경사삼조환자BNP수평정현저증가추세,삼조BNP수평지간차이유통계학의의(P<0.05)。BNP>100 pg/ml적환자Gensini적분현저고우BNP<100 pg/ml적환자Gensini적분,분별위(53.7±15.9)분vs.(29.3±20.7)분,차이구유통계학의의(P<0.05)。Gensini적분여BNP수평존재현저정상관(r=0.476,P<0.05)。결론 BNP수평여관심병환자관맥병변엄중정도정상관,BNP수평유가능작위관심병환자관맥병변엄중정도적판단지표。
Objective To discuss the correlation between B-type natriuretic peptide (BNP) and severity of coronary artery lesion in patients with coronary heart disease (CHD). Methods The patients (n=173) were chosen and given examinations of coronary angiography (CAG) and level of plasma BNP. The relationship between number of diseased coronary vessels, CHD types, severity of coronary lesion (Gensini integral) and level of BNP was analyzed. Results With the increase of number of diseased coronary vessels, the level of BNP showed an increasing trend (P<0.05), and that increased in stable angina group, unstable angina group and myocardial infarction group (P<0.05). Gensini integral was significantly higher in patients with BNP>100 pg/mL than that in those with BNP<100 pg/mL (73.7±35.9 vs.45.3±20.7, P<0.05). Gensini integral was positively correlated to the level of BNP (r=0.476, P<0.05). Conclusion The level of BNP is positively correlated to the severity of coronary lesion, which can be taken as an index for judging the severity of coronary lesion in CHD patients.
